@noahsussman Root cause almost always end up in finger pointing someone, or something done by someone. This decreases trust in the people and systems around the product. Quality then almost invariably go down at this stage.

@GeoffGlave @cdemwell well, currently fathering 2 specimens 17 months apart, still under 3yo, I'm not sure I've passed the hump where this statement is true. One seemed super easy to deal with, in retrospect. Two is not a 2x multiplier, but still.
