@MayaHolsen I advocate for refills on most discharge prescriptions for chronic medications. The downside of losing access to medications due to lack of follow up is significant.

Couldn’t agree with this more, esp with the uncertainty with COVID! Educate on the meds and give refills on the Rx. If possible, some type of meds to beds program will help with initial adherence as well! I am lucky to have the best #TOC team that patients love! @cmcpharmd

#TipsForNewDocs When a discharge med is really important (like clopidogrel post stent), give more than 30d supply. Yes, follow-up appt important to get refills, but missing doses of DAPT is worse than missing follow-up appts. #kittlesonrules

The providers I work with seem to really appreciate the work my students put into performing medication histories but it is great to see evidence like this!!

It turns out that having pharmacy students/interns perform pre-admission medication histories may reduce burnout for the other clinicians involved in a patient's care. Not a revelation, but it's the first time it's been looked at. pubmed.ncbi.nlm.nih.gov/32728956/ @JournalGIM
