New interventions are needed to identify, link and engage people in the healthcare system. We found that an HIE informed, social worker driven program implemented in the @GradyHealth ED improves linkage to care for out of care people living with HIV.

Only 57% of Georgians living with HIV were virally suppressed in 2017 and 2,595 invididuals were newly diagnosed. PrEP was prescribed to only 7% of people who could benefit. Proud to know the people actively improving these numbers. @JColasantiMD @Armstrws @j_sumitani
