@DrRissman When mine was about 15 months old, she stumbled and fell a few times in one day and I had the same thought 🤦🏻‍♀️ (she is fine!) I definitely find myself catastrophizing sometimes.

#PedsICU colleagues: talk to me about managing hypoxia with COVID pneumonia. Does it make sense to push our limits past where we would normally intubate for hypoxia/WOB? The trend is to follow the adult world with delayed intubation and tolerate hypoxia on HFNC/BIPAP.

@Medic_Russell Shaking my head as I recall the episode where the surgical residents couldn't diagnose a boy who clearly had Kawasaki disease. I can't blame them for not knowing a core pediatric illness, but perhaps they shouldn't be trying to take the place of an EM provider or pediatrician 🤦🏻‍♀️

It's interesting how different centers vary in management. Intrigued by the practice variations for #bronchiolitis and associated difference in length of MV. Correlation =/= causation, but pondering a nasal ETT the next time I see bronchiolitis (it's been a year!!)

@MedgibbonsCathy @brownam130 @SCCM @BasuND22 I'll admit my ignorance and say that all 3 PICUs I've worked in (training and as an attending) use full maintenance. One PICU would cut it to 2/3 if intubated due to decreased insensible losses. Maybe this is regional? I'm definitely questioning this practice now.

@brownam130 @SCCM @BasuND22 I’ve never worked in any PICU that actually gave 100% maintenance IVF. Standard seems to be 65-80% of that vol & even then it’s too much most of the time (provided enough cal. being provided). For me goal is no IV maintenance - enteral from the get-go & aim for reaching BMR ASAP

Great perspective with statistics that clearly lays out the reasoning for getting the COVID vaccine. It sucks that we're in a reality where there's no risk-free choice, but we can be informed and make the safest decisions possible.
