@jabberwock951 Recently had a 16 year old in an adult clinic, referred to paeds when 15 and referral rejected because “they’ll be 16 by the time we see them”

Has anyone come across paediatrics refusing to accept a 16 year old patient (who wants to go to paeds) and insisting they come under adult medicine? Happened to me today and I'm unsure how upset to be.

@amateuradam Do NHS waiting lists, food bank use, interest rates. Labour may have left debt but had the public services to show for It. Now we have terrible public services which can’t be stripped any further.

@reo_alvarez @FiFiELewis Put an auto reply on saying you aren’t taking up your post. It will settle your guilt while saving you from having to do anything about any emails!

@MedRegoncall1 Most disappointing thing was the consultant and the reg told them they’d not help because they’d not be able to do it any better. Also consultant shrugged shoulders when I raised about not doing it lying on a trolley and doing it in a wheelchair. They are being enabled.

@imacpherson90 It’s their “ego” that prevented them from asking any doctor for help/advice!

🔴Dear PAs, You shouldn’t perform abdominal examination on elderly patients whilst they’re sitting in a wheelchair in A+E. ⭕️You must put the patient on a couch/bed so you can feel for organomegaly/Ascites/Tenderness. ⭕️The surgeons would crucify anyone trying to refer a patient who was examined whilst sitting in a wheelchair.

@drandrewmackay Hospital managers asking for daily patient reviews in the afternoon fail to understand that creating a new plan at 4pm means an entire extra overnight stay vs creating it at 10am and actioning it during the day and discharging

@drandrewmackay Hope supervisor responded directly. It’s definitely important to prep discharges, it’s more important to be on the ward round to find out what’s actually wrong with them and it’s most important that if there were issues or concerns to raise at the time.

@Roddy_Neilson @NHSForthValley I hear intimidation of doctors in training by consultant haematologists is at record lows in the last few months.

@DrJlo29 had to do loads of these as resp FY1 for home oxygen assessment. Seems like a reasonable plan that someone being assessed for home oxygen is DNAR WLC

Actual medical plan seen today: 3. ABG on room air 4. TEP DNAR: ward based care WHY WHY WHY 🙈

@doctor_dru_ I don’t think agenda for change staff get days in lieu in return though. So they get paid more than usual for working a bank holiday whereas doctors don’t; but doctors get paid as usual plus get a day off later.

@harryoreilly @XelenX1 @OliviaMarieW7 But why would you ever work the same hours as a PA? That’s the point. That’s why we became doctors. Yeah the pay comparison is poor but it’s hyperbole which weakens other arguments.

@imacpherson90 @XelenX1 @OliviaMarieW7 You are completely missing the point. If I worked the exact same hours as a PA in Scotland it would take to the first year of consultancy to earn more.

I got 3 offers to study to be a PA whilst I was also applying to medical school. One fully funded. I turned them down because I wanted to be a doctor. I wanted career progression. I didn’t know about salaries. I love my job (most of the time) but my god, Silly me.

@XelenX1 @harryoreilly @OliviaMarieW7 Under 40 hours a week is unbanded. More than 40 is 1c regardless of amount of antisocial.

@imacpherson90 @harryoreilly @OliviaMarieW7 Also I've worked an unbanded job, 48 hours a week, in Scotland and was struggling to pay the rent during those 4 months. Which is pretty wild for a full time doctor.

@harryoreilly @OliviaMarieW7 Not what it compares though. Compares actual wage plus benefit of “progression”. Majorly misleading to suggest FY1s earns 29k a year (basically a lie) and makes strike arguments suffer.

@imacpherson90 @OliviaMarieW7 Those comparisons aren’t fair, compare it to a 40 hour week Monday to Friday 9-5. That is what PA’s work, and therefore if we worked that we would not receive banding supplement pay.
