A neonate was brought in this thermacol box for echo. Was surprised at first but later found that's it's an innovative and affordable solution with scientific basis as an incubator and means of transport for babies! Wonderful!

Before You Even Tour Homes ☐ I expect to stay in this city at least 5 years ☐ My job is stable and not dependent on bonuses or RVUs to feel safe ☐ My partner and I agree on timeline, budget, and lifestyle expectations Income & Cash Flow Reality Check ☐ The mortgage works on one income ☐ Housing costs are ≤ 25% of gross income (30% is a red flag, not a goal) ☐ I can still max retirement accounts ☐ I can still save meaningfully in taxable accounts ☐ I can take time off without financial stress Student Loan Integration ☐ My home choice does not derail PSLF strategy, if applicable ☐ I am not counting on future refinancing to make this affordable ☐ I understand how higher fixed costs lock in higher loan payments ☐ I have a written loan plan that still works after buying Down Payment & Liquidity ☐ I will still have 3+ months of expenses in cash after closing ☐ My down payment is not draining emergency or opportunity funds ☐ I am not using “doctor loans” as an excuse to overbuy ☐ I can handle repairs, taxes, and insurance without building up credit card debt The House Itself ☐ The home is easy to resell ☐ The layout and size fit my current life, not a hypothetical future ☐ I am not paying a premium for “dream home” features ☐ HOA fees and rules are fully understood and acceptable ☐ Property taxes and insurance are sustainable long term Lifestyle Creep Defense ☐ This purchase does not set an uncomfortably high baseline lifestyle ☐ I could live here happily even if income growth slows ☐ Future upgrades would be a choice, not a necessity ☐ I am not buying to “feel successful” Final Gut Check ☐ Renting for another year would not feel like failure ☐ I would make the same decision if housing appreciation were zero ☐ I am choosing flexibility over optics ☐ This house protects future options instead of shrinking them Hard rule: If buying the house makes you say, “It’ll be fine once I’m making more,” tread very very carefully.

Did MD med at LTMMC. Saw KEM med too while doing DM card there. LTMMC definitely over KEM for MD medicine. Faculty is good at both the places, but Sion is better with regard to clinical exposure, unit wise super speciality teaching and well organised academics including allied postings.

It’s difficult to compress with the sheath insitu and there are chances of recurrence. Take opposite side access followed by culprit sheath removal and then apply compression. Check with a JR from the other side. It’s also useful for balloon compression and stent deployment if needed.

Sri Lanka is a good travel destination in budget ❤️ I visited Sigiriya Rock, Dambulla Caves, Kandy, Nuwara Eliya, Ella, Mirissa, Unawatuna, Galle and Colombo, thereby covering all 3 - cities, mountains and beaches of the country. Skipped Jafna, Northern Sri Lanka and Trincomalee, maybe next time. 🤷‍♀️ Read this space for free itinerary and honest review 🧵

@MichaelMegalyMD Looks like aneurysm converted to enlarged pseudo aneurysm but no major perforation yet. That’s probably why stable hemodynamics. Wiring should not be an issue. Followed by DES or covered stent.
