Call me simple, but I’ve never fully understood the logic of chasing short-term results. For example, using steroid injections in an arthritic joint when the patient already knows they’ll eventually need a replacement. I can see the value if there’s a specific short-term goal like getting through a big event or an active summer but I doubt that’s the case in most scenarios. If me, I wouldn’t want steroids injected into my body I’d rather just have the joint replacement and move on with life. What am I missing? How often do you actually see a patient become asymptomatic after a steroid injection and enjoy years of relief? From what I’ve seen, the literature shows worse outcomes in patients who undergo multiple injections before eventually having arthroplasty. On top of that, steroids can harm bone health and have other neg physiologic effects. It feels like I’m missing something important here.

@generalorthomd The less common, tough cases are always hard and hard for everyone. I’ve seen many surgeons stop doing these cases with all number of justifications. Reality is that’s a disservice to patients and partners. Keep doing the hard stuff, though I know you will.

@generalorthomd My 8 yr old practices 5:30-7:30 M/Tu/Th and the season is 5 months long. And they expect parents to stay for the entire practice. Its a big ask for 3rd graders...

My son’s high school football team has practice M-TH 4:15-7pm, Fri 6am (they don’t have school on fridays) and mandatory 1:30-3 hour training in the summer. And they are wondering why more kids don’t go out for the football team.

@northwoods1980 For many of these conditions the non operative options are limited. Often PT/injections. Patients will drive this as they see injections as a “quicker” fix. Insurance will drive it as they require trial even in severe cases. The example above with lateral elbow pain is difficult

I think one of my biggest complaints with some orthopedic practices is their ridiculous use of steroid injections as if we have data to support so many of these injections around joints, etc. Where and why did this ever start? Is it a reimbursement situation?
