@kcrehabguy @ArthroscopyJ When we have some of the more prestigious institutions in ACL rehab/research quantifying their loading protocols with an “x” instead of sets/reps/some form of intensity can we be surprised. It’s probably as much talking past each other on what is loading as anything else.

New article from @ArthroscopyJ. I absorbed an ACLR that's roughly 12 weeks s/p and his 90 deg isometric test showed a 69% deficit. Sigh. Check out the highlighted spot. We. Suck. At. Loading. Then it manifests with terrible loading response.

Saw ♀️ Strongman competitor/powerlifter. Similar case few mos back, male Xfitter, convert to powerlifting. Both had crazy high volume session (30+ sets!) & worked thru, both seem to have distal glute max insertional tendinopathy/itis. Seen it @DMilesPT @DR_Ebner @DrQuinnHenochPT?

@Matthew_Rupiper I don’t *want* to like this but the accuracy of the tweet is spot on. Almost like the JOSPT article on trunk exercises. Low certainty in evidence, high certainty after a weekend course or because that is what the residency pushes.

We’ve done a few podcasts in TRT (EP 21 and 22 I believe), but may do a series on PEDs. People have assumed we are on tons of drugs for a long time, which is funny in its own right, but I find the TRT thing even more funny. Why? Because TRT in someone w/o low T does nothing.

@mike_exercise Do you have a citation for that? “Muscle tone” isn’t really a measurable construct. Stabilization exercises are typically as much motor learning/coordination as anything else. There are plenty of strong individuals that experience low back pain.

@DMilesPT I understand its not a panacea. But there is a strong correlation between people having back pain, and almost zero muscle tone, or ability to perform a stabilization exercise. Maybe there are many 12 pack ab people with back pain who don't come see us, but i think thats unlikely

@AlexisMLeveille It is all going to be based on professional opinion. I would say resistance training is perfectly fine for kids with a diagnosis of scoliosis. The angle “can” increase but it can also stay the same/regress. We can’t always make decisions based on the possibility of a problem.

@DMilesPT I think powerlifting is fine for adult scoliosis since progression is nearly nil past skeletal maturity (although there’s no studies I could find strictly on lifting heavy), but what’s your take on kids? Couldn’t find a single paper, but scolisosis can ⬆️ a lot in kids
