50 y/o with cysts in kidney and liver, read as PKD. GFR nl. no FH of CKD or PKD. Natera genetic testing negative for PKD 1 and 2. Are there variants not picked up by that panel? @fouadchebib @AskRenal

@AKronbichler @VelezNephHepato @jrseltzer Agreed! She underwent TPE earlier this morning and I started pulse steroids and CYC this afternoon. Will keep you updated about her course!

@DerekSLarson @VelezNephHepato @jrseltzer Okay, then you will need to consider full-blown AAV, and I would consider PLEX, higher doses of steroids and CYC as she is having a truly rapidly-progressing disease course

@AKronbichler @VelezNephHepato @jrseltzer TEE and cultures have been negative for this patient but she did require dialysis yesterday (creat > 5.5, etc).

@VelezNephHepato @DerekSLarson @jrseltzer Thanks for the swift response! Based on CRP (etc) I might nonetheless start with low dose steroids. The % of crescents triggers something in my therapeutic approach to such a patient
