Best within 2 hrs of exposure. 💉 HBV Immunoglobulin (HBIG) - Stock HBIG - Vaccinated workers with documented anti-HBs ≥10 mIU/mL need NO HBIG, regardless of source patient status. - Give as soon as possible- ideally within 12–24 hours, ineffective after 7 days. - Can be given simultaneously with HBV vaccine but at a different injection site 💉 HBV Vaccine - Vaccinate all healthcare workers pre-exposure — first dose post-exposure within 24 hours for unvaccinated workers. - Post-vaccination serological testing (anti-HBs) recommended 1–2 months after completing the series 🧪 HIV Testing - Use HIV Ag/Ab combination (4th generation) assay — detects both antibodies and p24 antigen ✅ - If source has HIV or cannot be tested, retest exposed worker at 4–6 weeks and 3–4 months. - Any symptoms of acute HIV → immediate evaluation. 🧪 HBV Serology - Source patient: Test for HBsAg to determine infectivity - Exposed worker: Test for anti-HBs to determine immune status (protective level ≥10 mIU/mL) 🧪 HCV - No PEP exists for HCV, but early detection enables effective treatment - Post-exposure assessment should include HCV evaluation for all needlestick injuries - Baseline HCV antibody + ALT; follow-up at 3–6 months 🔴Other important points -Source patient rapid testing — HBsAg, HIV (rapid), HCV; drives all PEP decisions - Mandatory incident reporting - There should be a needlestick injury nurse incharge designated in the ED who is available 24/7 for early implementation of this protocol.
