@leuprolide @princess_worms cortisol levels vary a lot throughout the day and days based on stress etc and it's possible to have something numerically in a normal range on a test; but still have symptoms consistent with not enough which are alleviated by exogenous hydrocortisone

@leuprolide @princess_worms diagnosing mild adrenal insufficiency (from a mildly clapped out enzyme in the synth pathway or from signaling stuff upstream) is harder because unless you have a stark case it seems like you can have a case where you make enough to live but not enough to cope with lotsa stress

@princess_worms @leuprolide (20mg / day hydrocortisone is replacement level; dont go above that on average. also if you take hydrocortisone for a while it downregulates your adrenals so if you stop taking it suddenly you can get adrenal crisis (BAD!) because they cant ramp up fast enough, please be careful)

@princess_worms @leuprolide honestly i wouldn't diagnose it based on that. they often do give IV steroids or postop steroids too. i would look at labs and also like...day to day symptoms and ability to cope with stress and get up in the morning (vs needing hours to get up and spending weekends convalescing)

@princess_worms @leuprolide and also going into surgery (purely hypothetical; and definitely not something happening in 60 hours) and recovering without knowing i need stress dosed steroids would have been.....less than fun

@princess_worms @leuprolide that mf straight up saved my life; I literally lost my entire 20s to fatigue from this bullshit adrenal insufficiency thing. apparently HRT makes it worse because estrogen increases cortisol binding globulin or whatever.

@princess_worms @leuprolide the funniest thing is he ran a steroid panel on me and my 17-OHP was below detection and from that and symptoms he clocked i had clapped out adrenals and was salt-wasting (and wasnt surprised when i showed him i kept salt in my bed. havent had to use it since he put me on fludro)
