Maybe in some cases, but I sold health insurance before ACA and at that time, with rare exception, you could still get insurance, but there was an exclusion period, usually a year before your pre-existing would be covered. It was to protect the drive-by policy scammers.
Yes, your rates were sometimes dependednt upon PEC, but then you're in a high-risk pool.
ACA took those protections away from insurance policies, so now everyone pays higher.

Not sure that this is an efficient model, requiring everyone to pay for services that they won't/can't use. Insurance companies are REQUIRED to provide the benefits, so they need to charge for it.
It stopped becoming a risk mitigation strategy, which is the entire point of "insurance" and people started expecting it to cover everything. It's now just financing.
"Free" preventative services means that insurance pays the entire bill. You expect them to eat that? No, it all gets charged back in premiums.

The net effect was higher premiums/deductibles for healthy/high middle income, but better coverage/less junk insurance.

Here's the plan...get government out of the relationship. Sell across state lines. Permit minimal plans for catastrophic-only coverage, high-deductible plans, let people choose their coverage levels.
But midwits don't want to hear that, you still believe that there's an undiscovered, magical government solution that will make someone else pay for all your healthcare and only rich people will foot the bill.
