“Josh Pirestani Let’s stop pretending. Independent pharmacy owners aren’t “struggling.” They’re being systematically screwed. Primary wholesalers lock you into contracts, dangle rebates like carrots, then tighten margins so hard you’re basically financing your own survival. PSAOs talk about “leverage” and “strength in numbers”… But contracts get worse. Reimbursement gets thinner. Transparency disappears. Where’s the strength? PBMs? They set the reimbursement. They define the performance metrics. They audit you. They fine you. They own the mail order. They own the specialty pharmacy. They own the data. And somehow they’re the referee too. You buy high. You get reimbursed low. You wait to get paid. You hope there isn’t a clawback. You pray a generic doesn’t spike overnight. Owners are: • Using personal savings to float payroll • Taking no salary • Working 12-hour days • Signing contracts they can’t negotiate • Competing against companies worth billions And then the industry panels say: “Pharmacies need to adapt.” Adapt to what? Negative margins? Contracts you can’t see? DIR replacements? Point-of-sale games? Everyone in the supply chain gets paid first. The pharmacy owner gets what’s left. If independent pharmacy disappears, it won’t be because owners weren’t good enough. It’ll be because the system was designed to extract from them. MRIs can cost $600 at one hospital and $6,000 at another, and patients are kept in the dark. President Trump requires price transparency, but most of the medical establishment ignores it. Our fix: if patients aren’t told the price upfront, providers don’t get paid — no transparency, no pay.
