- broad antibiotic coverage. Don’t be shy!
- classic skin findings are mostly seen in subacute endocarditis. Don’t overly rely on these
- TTE is always helpful but still misses a significant number of cases

COVID Rule Out - 45 ym with fever, hypoxia and dyspnea. Placed in an isolation room from triage. Our amazing resident remembered to #echofirst and made the diagnosis of aortic valve endocarditis (slowed video for clarity). Abx & proper dispo. Reduce anchoring bias with POCUS

- normal retic count and Hb do not rule out “real” pain
- do a thorough extremity and joint exam. They are high risk for septic arthritis and osteo

Don’t forget valvular causes! If comfortable with echo and there is critical AS —> phenyl is pressor of choice
- severe MR —> CT surgery or interventional
- severe AR —> try and visualize aortic root for dilation
