@GmOrr9000 It should be mandated that unless disabled, each medical school graduate should have to practice medicine FULL TIME for 15 years (minimum) or have to refund the Federal Government for the cost of tuition.

#1 reason for MD shortage is Med schools admitting students who aren’t going to practice. Only 60% of med students today plan on practicing clinical medicine. Med Schools need to focus on admitting students who want take care of patients, not just get their MD.

@davidcarr333 Definitely not ideal but at least EM is adept at building rapport and trust swiftly, and similar to ICU sees patients at their worst moments, and can help give that perspective to families or patients who may not have that insight. So our experience is valuable. Not easy place.

@davidcarr333 Agreed. In my intensivist role often am confronted with similar conversations during admits & rapid responses. While not ideal, and wish convos were sooner, on other hand at least CCM has experience with EOL discussions & in lieu of pal care can provide some guidance for pt/fam.

I find myself wondering why as an ED physician, I am often the one clarifying code status on cancer patients towards the end of their journey. These are important discussions and considerations that should occur upstream by their longstanding trusted treatment team.
