**HELENE DONATION UPDATE** Many people have been asking where I recommend sending donations. My advice is to go very big or very small. Without further unpacking I will acknowledge there are strengths and weaknesses to both. Very big would be orgs like Red Cross, state government, or United Way. Very small would be county-level or smaller. For very small destination, I've been sharing Burke Rescue as a site, my own rescue squad. While I still wholly endorse donations there, I think Burke is pulling out of this. I have lots of colleagues in Mitchell County, which has been harder hit than us (think Spruce Pine, which has been very much in the news). They recommend the AMY Foundation as a good county-level recipient of disaster funds, so I'm switching my "local" rec to them. The benefit is that AMY (Avery-Mitchell-Yancey) also includes Burnsville (Yancey), another heavily hit rural site very much in the news. When you hear about rural places in NC that are still compromised, it's places like this. It’s about one week to the hour since my son Noah and I chainsawed our way through massive poison ivy vines and trees out of the Lost Corners neighborhood of Morganton NC. At that time rain was pouring sideways and leaves were flying upwards. This leaf is about 8 meters above the deck, out our second story window, still suspended by two gossamer spider threads. I think it’s a powerful metaphor for the ongoing tenuousness of our situation and communities still in suspended and compromised circumstances, held together by gossamer threads of community and faith. But it’s also worth pointing out that spiderwebs, which appear fine and fragile, are stronger than steel when compared by weight. Overall, this still-suspended leaf one week out is an awe-inspiring testament, one week out, to the power of nature over us still… as readers of _Wilderness EMS 1e_ know, the anthropologically-driven definition of wilderness medicine and wilderness EMS. #thisisanthropology #thisispublichealth #HurricaneHelene #resilience #leavesinthewind #WEMSText @WEMSText

-: CWEMSE Day 28 :- [9/21/2024, posted late on 9/22/2024] Today’s Travel: 1352 km Total Travel*: 15,003 km* Knot of the Day: stopper knot BARKLEY MARATHONS FALL CLASSIC Our final day of the Externship was spent in collaboration with the Appalachian Mountain Rescue Team, providing medical support to The Barkley Marathons Fall Classic. This remarkable 50 km ultramarathon is one of the most famous & challenging in the world. The route, which changes every year, must be navigated by racers without GPS, and which they only receive access to the morning before, includes about 3600 m of elevation change on each loop. More info on the marathon series: youtu.be/NDZdsqbcGTU?si… This really was a perfect capstone experience to the Extern’s month & gave them real-world experiential education around event medicine and austere/wilderness medical care. And for sure the grueling characteristics of the Barkley provided plenty of experience! We were positioned at the top of a truly gnarly series of switchbacks up Bird Mountain, & just before the Pillars of Doim. With a name like that what could go wrong? A particular treat for us was meeting & getting to walk for a while with Laz, the famous course designer and founder of the Barkly! Following this, and a late night debrief, the Externship ended as it so often does: hugs and farewells in a remote forest/park parking lot, with Externs and Seth reminding each other that this is not the end… it’s only the beginning of a beautiful collaboration together. We challenged ourselves to the limits of our new knowledge and the Externs truly excelled in their roles. BRATT class got rave reviews from them!. Kudos to course faculty MD-53 Chris Davis (EX13), MD-68 Abbie Wehner (EX20), MD-1 Seth Collings Hawkins, and Chris Peluso (and especially Peluso for cooking all the meals!) Immediately upon wrapping up the course at 1500, after bidding adieu to Poplar Creek Flower Farm and Pilot Mountain, we launched for our final destination: Frozen Head State Park (TN) and medical support of The Barkley Marathons Fall Classic Ultramarathon there. Stopping on the way for food supplies at Ingles, the Externs got a little nutritional primer from Chris “Food is Fuel and Medicine” Peluso. Naanep reported “I haven’t been pimped** so hard all month!” Hahahaha… our interim destination tonight was Morganton, where we hit up perennial Externship favorite Mountain Burrito. We have a tradition of holding our graduation ceremony a few days before actual graduation, since the final weekend is so jam-full of operations and adventures. So tonight, on Day 26 in the Poplar Creek Farm meadow, we celebrated the amazing accomplishments of our two Externs this year, MD-75 Laura Westneat (The University of North Carolina at Chapel Hill) and MD-76 Bret Naanep (University of Illinois Chicago). On hand to celebrate were the entire BRATT class as well as Extern Graduates MD-53 Chris Davis (EX13), MD-62 Wooten Jones via FaceTime (EX18), MD-68 Abigail Wehner (EX20), and MD-71 Kara Hatlevoll (EX22). Cake was delivered by Den Mother & Number 1 Volunteer Kelly Collings Hawkins. During the ceremony the Externs not only receive their plaques but also get to present the Golden Carabiner Award to the person who was most significant to their learning for the month. They presented it this month to Wooten Jones, our Associate Director for Coastal Operationsm who is responsible for the amazing experience that is the #CoastalModule. Today we discussed major roles in a technical rescue incidents, learned how they move patients through high angle terrrain, worked on communication hacks, and worked on raising and lowering an attended patient, getting chased off the mountain just at the end by an incoming thunderstorm. We repositioned to the Farm for a didactic session on mountain rescue injury research from the Wilderness Medical Society scientific journal. This was an intensive day of training with two dozen students on all elements of delivery of medical care in an austere, remote, or wilderness environment. We were particularly privileged to be joined by the national medical director and EMS director of the U.S. Forest Service, who joined us for this event held in Pisgah National Forest! After this 100% hands-on training, we all returned to Steel Creek Park and Campground for Boulevard Barbeque, drinks, catching up with old friends, making new friends, and mentoring and grey/parallel/hidden curriculum work deep into the night! The Seminar this year was attended by every EMS Fellow and Wilderness Medicine Fellow in the state of NC, who are the primary target audience. But we were also delighted to include paramedics, nurses, medical students, and resident physicians this year. After the training we repositioned to Hillman Beer - Morganton for more opportunities to connect and explore wilderness EMS topics in a more relaxed setting. They then spent the evening at the Burke County Rescue meeting getting credentialed in traffic incident management (part of the NC Tech Rescuer cert and acknowledgement that a disproportionate number of injuries and fatalities on SARS and WEMS involve vehicles). -: CWEMSE Day 16 :- [9/9/2024 posted late on 9/10/2024] Today’s Travel: 620 km Total Travel*: 9,448 km* Knot of the Day: square knot BURKE RESCUE/SOUTH MOUNTAINS DAY The Externs spent the morning with Burke County Rescue and SAR. Learning how massive an operation this is, sustained entirely by volunteer time and skills, was an incredible learning experience. This is the sort of thing that doesn’t land in a PowerPoint presentation, but is best understood and appreciated experientially. From there the Externs repositioned to South Mountains State Parks to train and respond in the largest state park in North Carolina. They patrolled the park by ATV and foot. We are indebted to Ranger Ian Magill for spending the afternoon teaching us about SAR and wilderness EMS in remote areas of NC. The Externs then returned to Morganton and the cultural experience that is Little Guatemala for dinner and debriefing.
