H. pylori treatment has become a lot less “one-size-fits-all.” With rising antibiotic resistance, the old habit of reaching for clarithromycin based triple therapy no longer works as reliably as many of us were taught. Recent guidance instead recommends 14 day bismuth quadruple therapy as preferred first line treatment for treatment naive patients, with rifabutin triple therapy or vonoprazan-based therapy as alternatives when needed. What stands out to me is how much this reflects a bigger shift in medicine: choosing regimens not just by habit, but by resistance patterns, tolerability, and likelihood of actually getting eradication right the first time. And an important reminder, every treated patient needs a test of cure. 158,850 adults are projected to be diagnosed in 2026, and 55,230 lives may be lost @AmericanCancer . Increasing screening to 80% could reduce colorectal cancer diagnoses by 22% and deaths by 33% by 2030. When detected early, about 89% of patients survive 5+ years, compared with 16% when diagnosed at an advanced stage. Screening could also help reduce healthcare spending by $14 billion by 2050 @CDCgov . We often choose between apixaban and rivaroxaban for acute VTE almost interchangeably. But a new randomized trial (COBRRA, @NEJM ) suggests the choice may matter more than we think. Among 2,760 patients, clinically relevant bleeding was lower with apixaban (3.3%) vs rivaroxaban (7.1%), with similar VTE recurrence.
