@Rfisher41 @DrDiGiorgio @Chillcatser @MaryBowdenMD No doctors office I've ever worked for has done this. It's a good idea if the money goes to the right place. But they don't charge for every message you send, either. Just the ones that end up with a different treatment plan.

Imagine if every passenger on an airplane had a direct line to the pilot. That’s what medicine has become. We have embraced frictionless communication and that not a good thing. With things like Epic Chat, every member of the healthcare team has immediate access to the physician. Now there’s a constant stream of minor questions. It’s discouraged people from thinking critically. “Just ask the doctor,” has replaced any sort of clinical reasoning. When I was a resident, we had an answering service. Anyone who wanted to reach the doctor needed to go through a third party. This provided triage and accountability. If we were getting called in the middle of the night for stool softener orders, there was a record of that. A little friction in communication is a good thing.

@Rfisher41 @DrDiGiorgio @Chillcatser @MaryBowdenMD That's not what I'm saying. I fully support doctors charging for this. Using Epic message to replace an office visit should be charged for. I'm not sure how the back end for this works because this is the only office I've ever seen do it.

This isn't accurate. Most people have deductibles, anda higher out of pocket max. During the deductible phase, you absolutely pay out of pocket and should be able to price shop, and count against your deductible the best price you can find. Where the shopping analogy falls apart is AFTER you hit your deductible and OOP max. Then you can spend other people's money on your care. Distorting the entire market

We’ve said it once, we’ll say it again. You don’t compare prices for healthcare because, in the vast majority of cases, you aren’t the one paying for it. If you don’t have to pay for it who cares what the prices are. What would happen to prices at the grocery store if shoppers were paying with someone else’s credit card?

Yes, you still need to eat even if the macros aren’t perfect and you feel weird about your body right now. No, you can’t remove your own IUD. Give it more time or at least let the doctor do it. Yes, you should still wash your hair even if you’re just going to the gym and don’t even have a gym crush anymore. No, you can’t take a mental health day to go get a tattoo. And other internal conversations I’ve had with myself before 10 am.
