🚨 New ESICM Guideline Published! 🚨 We’re proud to announce the release of the European Society of Intensive Care Medicine (ESICM) Clinical Practice Guideline on Fluid Therapy – Part 3: Fluid Removal at the De-escalation Phase (Intensive Care Medicine, Aug 2025). This publication is of particular significance for the International Fluid Academy (IFA), as three members of our Executive Board — Prof. @Manu_Malbrain, Prof. Xavier Monnet, and Dr. Adrian Wong — contributed as co-authors. 🧠 How Does Sodium in IV Fluids Affect the Brain After Stroke? 🧪 A new preclinical study published in Scientific Reports explores how fluid sodium concentration impacts the brain, lungs, and kidneys in acute ischemic stroke (AIS). Using an experimental stroke model in rats, researchers compared four IV fluids: 🔹 Isotonic saline (0.9%) 🔹 Hypotonic saline (0.45%) 🔹 Hypertonic saline (1.5%) 🔹 5% Glucose 💡 Key Findings – Brain Tissue: ✅ Isotonic saline preserved extracellular matrix integrity, with significantly higher levels of versican and hyaluronan. ❌ Hypertonic saline triggered inflammation: increased IL-1β, VCAM-1, and ZO-1 expression, indicating endothelial activation and potential BBB disruption. Hypotonic and glucose-based fluids showed mixed effects, with glucose associated with lower inflammatory markers but not with matrix preservation. 📊 These findings suggest that isotonic fluids may best protect brain tissue post-stroke by maintaining osmotic balance and minimizing inflammatory responses. 💡 Fluid Management in Acute Brain Injury – Why One Size Doesn’t Fit All Did you know that fluid strategies in traumatic brain injury (TBI) are often borrowed from general ICU practice — and that this might be doing more harm than good? 🧠💧 In their latest Intensive Care Medicine editorial, Niels Van Regenmortel & Mathieu Van der Jagt dissect the evidence and challenge common assumptions: 🔹 Resuscitation: Balanced crystalloids may help in general ICU cases but could worsen cerebral edema in TBI. Isotonic saline remains safest — but watch for hyperchloremia. 🔹 Maintenance: Hypotonic glucose solutions can be risky in brain-injured patients. 🔹 Alternatives: Balanced hypertonic solutions show promise, but more research is needed. Albumin (hypotonic) is discouraged in TBI. 🧠 IV Fluids in Brain Injury: Salty or Sweet? The Ongoing Debate Choosing the right fluid in patients with acute brain injury remains a delicate balance. In their latest editorial in Intensive Care Medicine, Van Regenmortel & Van der Jagt revisit the evidence for resuscitation and maintenance fluids in neurocritical care. 🔹 Saline vs. Balanced Solutions: Meta-analyses show balanced solutions slightly reduce mortality in general ICU populations—but TBI patients may be the exception. Evidence suggests hypo-osmolality in balanced fluids like Plasmalyte may increase mortality risk in TBI. 🔹 Chloride Load vs. Kidney Safety: While NaCl 0.9% remains the go-to for cerebral protection, hyperchloremia's renal effects shouldn't be ignored. Balanced hypertonic solutions might reduce chloride burden while preserving ICP control. 🔹 Maintenance Fluids—Glucose Dilemma: In TBI, hypotonic glucose solutions risk cerebral edema. Yet, isotonic glucose–saline combinations remain underexplored.
