If you're thinking about healthcare spending in the US, one fact worth noting About 9% of healthcare spending goes to physician compensation Another 9% goes to nurses Yes, doctors and nurses get paid more in the US than they do in other countries and yes, a small proportion of physicians really do get paid a lot But I've never thought we're going to solve our healthcare spending by going after physician and nursing salaries Not enough there -- and slashing physician or nursing compensation would be a great way to demoralize the core of the US healthcare workforce

Earlier today, the @FTC voted 3 to 2 in favor of banning non-compete clauses in all employer/employee contracts, with no exceptions. Along with the vote, the FTC has issued a final rule, finalizing it proposal from January 2023 to ban non-compete clauses. The effective date of the final rule is 120 days after it is published in the Federal Register. It bans new non competes with all workers, including senior executives**, after the effective date. For existing non-competes, the final rule adopts a different approach for senior executives than for other workers. For senior executives, existing non-competes can remain in force. Existing non-competes with workers other than senior executives are not enforceable after the effective date of the final rule. ** Senior executives are defined a worker who is in a policy-making position and who received from a person for the employment total annual compensation of at least $151,164 in the preceding year. A policy-making position is defined as a business entity’s president, chief executive officer or the equivalent, any other officer of a business entity who has policy-making authority, or any other natural person who has policy-making authority for the business entity similar to an officer with policy-making authority.

What’s missing is these policy recommendations to congress have largely driven private practices out of business. Hospital systems run medical practices at a loss, but are able to cross-subsidize physician salaries by making a profit on the inpatient side while securing high margin referrals, ancillaries etc. Yet in the end, consolidation costs the system more overall. MedPAC’s rationale is siloed & flawed and has created/perpetuated this problem.
