Do you why its so hard for a primary teacher to own a used Toyota Land Cruiser compared other professions. TRUE! “Best interests of the child” applies clearly to a born child, not directly to a fetus as an independent legal decision-maker in most clinical settings. In obstetrics, the competent pregnant woman, not the fetus, is the patient with autonomy( decision powers). That is why a mother can refuse a cesarean section even if fetal distress is obvious, besides forcing surgery is ethically & legally far more complex than treating a born child But in practice, ....How will you pay for this treatment??.....if the parent says no....things get complicated, unless of course, patient is insured. And there is a willing other person to take care of the child.

Some time ago, I begged someone to consent to a cesarean section. There was a clear fetal indication for CS. She declined. She ultimately gave birth normally, but the baby didn’t score well. She even celebrated that she had been coerced into a CS but ultimately gave birth normally, that is what she told fellow mothers in labour. A year later, I met her, the child hasn’t even sat without support. I felt sad. I couldn’t even talk with her. Some things drain you emotionally.

@wooden_don @kipkoecheruiyot Patients make decisions based on many things: FEAR, MONEY, PAIN, CULTURE, RELIGION, PRIOR BAD EXPERIENCES, or a different understanding of the risks.... And if your communication skills are not up there, you may never know why!

@AlanBesiime @kipkoecheruiyot Dr stated there was CLEAR fetal indication for CS. Some people think having birth via CS makes them less of a woman (for religious/cultural reasons). It is the doctor's job to provide informed options and then act on the patient's wishes NOT win hearts and minds.
