I was prescribed amphetamines in grade school, no nicotine then. Studies show nicotine improves sustained attention and concentration in non-users with attention deficits (like low-attention groups or ADHD traits), not just withdrawal relief. It hits similar brain pathways. Dependence risk is real, but effects/benefits aren't imaginary for everyone. I also ENJOY using it in this way and I'm not interested in stopping. I wanted to stop smoking because my health was declining and it was literally killing me. Not any more. And I'm not going back to amphetamines to function normally. If there are people who truly want to stop, but can't, you should continue helping those people meet their goals. For the rest of us, you shouldn't be telling us what we are allowed to enjoy or find personal benefit in continuing to use with little to no residual harm in the risk/benefit analysis.

Especially, if you notice a reduction in distraction with each dose?

Yes, I know I can but due to a mixture of addiction and choice re benefits I choose and to some degree are compelled to continue. Given I receive actual benefits from it I continue in a moderated way. Smoking, vaping and other nrt dependencies have some simularities but the balance of positives and negatives are different and given this the ability to not do it is not the same either. Different addictions have different profiles. A heroin addict could make the same argument

Yes, I enjoy one or two coffees a day even though it messes with my tummy. Vaping is similar in some respects like you say but there are some differences too. The balance between positives and negatives in one reason. Another matter is the degree of choice. As I preposed the positives are illusionary, the negatives understated and given all this the degree of choice is minimal. Its much better than smoking but still better without. Ive been going on about it to highlight that there are additional ways to be totally free of any of it. Thats providing even more choices for those who feel inclined to look into it. I think such information and the additional options such info opens up are very valuable and sadly not so widely known :)

Abstinence is the safest option, just as avoiding sex completely is the best way to prevent STIs, and avoiding drugs entirely is the best way to prevent overdose. But medicine is not practised in a fantasy world. It is practiced in the real world, where people do not always stop risky behaviours on demand. That is precisely why harm reduction exists. For smokers who cannot or choose not to quit nicotine, moving away from combustion matters. Smoking kills primarily because of burning tobacco and inhaling the resulting smoke. Pretending there is no meaningful difference between combustible cigarettes and lower-risk nicotine alternatives is not caution; it is negligence dressed up as purity. A physician should be able to hold two ideas at once: 1. Not starting is best. 2. For people who already smoke, reducing risk is still a medical good. Rejecting tobacco harm reduction while embracing drug harm reduction or sexual harm reduction is not consistency. It is ideology. I urge you to look deeper and compare your approach to real-world experiences, such as the 1.3 billion smokers and 8 million premature deaths that occur annually. If it was unattainable, then why do these numbers still exist?

The safest alternative to smoking is to not smoke. That is the option. And you are behaving as if that is an unattainable ask, but it’s not. Any form of ingesting toxic fumes into your lungs is not safe. Period. To suggest it is, as a physician, is absolutely reckless.

Another doctor who makes uneducated and misleading medical claims: "University of New Mexico pediatric pulmonologist Dr. Michael DiGirolamo said among other claims, that the chemicals in vaping products can release shards of nickel, lead and other metals into the lungs and can expose the body to formaldehyde, a chemical known to cause cancer and other respiratory issues. @RobertoSussman @LanceChurchill Follow-up question: If people clear these metal oxide nanoparticles just fine in cooking, atmospheric, etc. exposures, why are some making this same or lower exposure from vaping such a big deal when, as far as I know, levels do not exceed pharmaceutical inhalation limits?

Claim they cleanse the lungs, reduce stress, etc. They say " oh I'm not vaping, this is a personal diffuser". Ingredients show pg,vg, artificial flavor. Oh Ok👍
