Wow! Glad I lived long enough to see this. Inspiring! Here’s a photo of how it was back when I first started out as a young design engineer in the early 70’s. It resonates that are probably many other treatments and potentially impactful drugs that won't be included in the Standard of Care as they haven't gone through phase 3 clinical trials.  3. Personalized medicine: organoids, digital twin, faster cycle from biopsy -> identifying targets -> creating customised solutions, such as mRNA vaccines, custom Antibody-Drug Conjugates (ADCs), and novel ways of triggering one's immune system.  4. Improving measurement: Currently, the standard PET-CT method does not allow weekly measurement of disease progression. Identifying a superior method to allow weekly measurement of progression could be instrumental in cutting down the time it takes to find a drug the body respond to. This is important not only for lung cancer, but maybe for other cancers and diseases as well. This goes back to another study where they found that NSCLC patients receiving immunochemotherapy before 11:30 a.m. lived significantly longer than those treated later. pubmed.ncbi.nlm.nih.gov/39983330/ The idea seems to be simple … Our immune system is regulated by circadian rhythms, which influence immune cell trafficking, hormone levels, and metabolic processes throughout the day. Administering immunotherapy when immune activity is naturally higher or more receptive could help the drugs work better. Once all the data have been gathered together, we can figure out how beneficial this is and whether all patients benefit equally, or whether the effect varies by things like age, performance status, or tumor type.

Instead, he puts it in a trust. Borrows $500K against it. Gives it to his kid. Borrowing is not income. So no tax. Uses lifetime exemption to avoid gift tax. He lives on loans too. Never sells. He dies holding the asset. Kid inherits at stepped-up basis. IRS gets $0.
