For far too long, rural Iowa has been ignored. As Governor, I will stop sending economic development dollars to multinational corporations and bring real investment back to our small towns and rural communities.

EXCLUSIVE Colorado Teacher REFUSED to allow a 7th grader to present her pro-life slam poetry submission because it’s “offensive” and might make kids feel “unsafe.” Some examples of accepted topics in the class are slamming the 2nd amendment, mocking Jesus, and lgbtq rights. Staff admitted that the poem met all the requirements however couldn’t be read out loud because it’s “politically charged.” The teacher also initially tried kicking this 13-year-old girl out of class during the poem presentations but allowed her to stay after pushback. We spoke with the mother and daughter who shared their story and with us and why being pro-life is so personal to them. This happened at Drake Middle School in @JeffcoSchoolsCo.

Quietly Replacing American-Trained Physicians The Incremental Shift in Teaching Hospital Staffing: A Progression Toward Foreign-Trained Influence Abstract This analysis examines the gradual increase in foreign-trained physicians (international medical graduates, IMGs) in U.S. teaching hospitals over the past 30 years. From negligible representation in the 1990s to current levels of 50 to 80 percent in many residency programs and 20 to 25 percent in faculty roles, the shift reflects structural incentives rather than deliberate design. Data from national reports illustrate how this progression alters selection, mentoring, and leadership, potentially disadvantaging domestic talent in medical education and practice. Introduction U.S. teaching hospitals are the cornerstone of medical training, responsible for residency programs that produce the nation's physicians. Thirty years ago, in the 1990s, foreign-born or trained staff were a small minority in these institutions, comprising less than 10 percent of residents and even less in faculty or leadership positions. Domestic graduates dominated, with American-trained faculty selecting and mentoring the next generation under a relatively uniform system. Today, the landscape has transformed. IMGs now fill significant portions of residency slots and junior faculty roles, influencing the pipeline for future doctors. This progression is driven by economic pressures, visa programs, and policy gaps, leading to a self-reinforcing cycle that prioritizes cost and availability over domestic investment. Methods Data were compiled from the Association of American Medical Colleges faculty reports (1995–2025), National Resident Matching Program match results (1995–2025), Federation of State Medical Boards licensure summaries (2026), and Journal of Graduate Medical Education analyses on IMG representation (2025). Percentages reflect national averages and program-specific distributions in academic medical centers. Results The shift began in the late 1990s with Step 1: Staffing the gaps. Teaching hospitals, especially lower-tier and community-based programs, faced challenges filling residency positions amid capped Medicare funding for slots since 1997. U.S. graduates preferred prestigious programs or higher-paying non-academic roles. IMGs and H-1B holders stepped in, accepting lower pay, longer hours, and less bargaining power. By 2005, IMGs comprised 20 to 30 percent of residents in internal medicine and primary care programs. By the 2010s, Step 2 emerged: Culture and decision-making shift. Resident classes in many programs reached 50 to 80 percent IMG composition. Senior residents and junior faculty began to mirror this demographic. They mentored incoming classes, and selection committees incorporated more IMG perspectives. Institutions grew accustomed to fast-track foreign paths, as many operational leaders had navigated them themselves. Faculty representation rose to 15 to 20 percent IMG by 2015. Step 3 followed in the 2020s: Leadership follows. Over 10 to 15 years, some junior faculty advanced to program directors, division chiefs, and department chairs. While not a majority (IMG program directors remain under 15 percent nationally as of 2025), their presence normalized provisional licensure and reduced resistance to importing more foreign-trained staff. The system self-reinforces: more IMGs in training leads to more IMGs in leadership, which eases justification for further IMGs. Step 4 seals the progression: The profit layer. Hospitals benefit from lower payroll (visa-dependent staff accept less), higher compliance (less likely to challenge protocols or billing), and sustained reimbursement. American-trained doctors, burdened by debt and options, are harder to retain or control. From near-zero foreign-born influence in leadership 30 years ago to a serious 20 to 25 percent in faculty and 40 percent in residencies today, the change is real and accelerating. Discussion This progression is not conspiratorial but incentive-driven. Economic pressures and policy gaps create a pipeline rerouted through foreign-trained gatekeepers, who have limited motivation to prioritize domestic talent. The result disadvantages U.S. graduates, perpetuates two-tier standards, and shifts control in medical education. Policy reforms should focus on expanding domestic slots rather than imports to restore balance. Citations (APA) Association of American Medical Colleges. (1995–2025).
