It was a great honour to perform and transmit a live case from Victoria, British Columbia, Canada for the AICC 2025 meeting held in Chennai, India. We performed a successful complex retrograde CTO case in a young patient displaying the algorithmic approach to challenging microcatheter passage; tip-in technique as an alternative to routine externalization; and important steps on how to minimize complications in retrograde CTO PCI. We were able to use a hybrid DES-DCB strategy to treat this young patient.
