Imagine someone not in your specialty reading your notes - spell out specialized acronyms at least once (smart phrases can help).
Fewer abbreviations = better communication = better care!

No - looks most like Lambls excrescences (especially in absence of pathologic AV - low risk for IE).
Would review with old echo's when available.
