Condolences to #EPS this morning on the loss of 2 officers. A terrible loss for the whole city.

@AlisonKabaroff Hahaha thank you 😅 I have residual trauma from an event where we had to bag a tubed patient for 30 hours and ran out of narcs to the point we were using ativan/morphine/ketamine to keep ‘em down, so now my brain is always in a chronic ‘worst case Ontario’ mindset 🥲😂

Halp. Can someone actually go in and out of WPW? I can’t make heads of tails with the insane variability over 9 months of ecgs where two from months apart have D waves but then others in between and before/after do not. In fact some are NSR, 1° blocks and some are 2° type I.

@jesssicrap Intermittent pre-excitation is a better prognosis. And SCD in WPW is thankfully rare.

@AlisonKabaroff I’m shook reading the recent cards report; they somehow didn’t have access to the previous ecgs, or records of the ablation a couple years ago done at the Maz and don’t seem particularly concerned, meanwhile I’m all: please don’t drop ded we’re closer to the North Pole than an ER

@jesssicrap Also, some people will have a classic WPW pattern on their ECG but never actually go into a tachydysrhythmia. WPW pattern vs WPW syndrome.

@jesssicrap Yes. You can have temporary accessory pathway activation. Sometimes the ECG will show a WPW pattern and sometimes it won't.

Ppl love to make fun of ED docs for ordering what they believe are inappropriate tests, but in reality the cohort of ppl in the ED who overwhelming are the most persistent and demanding of actually inappropriate tests/imaging are physicians who bring in their loved ones.

@DanBradnam @_JaceMullen Nope. We put the line in the internal jugular not the external. Is the patient alive? Great, I can move the line later.

@_JaceMullen I wonder if they could still feed a guide wire through the cathalon and just continue with the line from there?

I’d like to take a second to apologize to all the EM and crit care docs for the right sided EJs I placed in patients you probably wanted to line. I didn’t know better then but now that I do know better I will probably still do it again, but only if I have to.

CBC posted an article today roughly stating virtual care did not drive patients to emergency departments as predicted - and then took it down. Anyone see it?

@aaronjohnston @PfParks A recent example is a patient with sepsis. Seen in the WR. Source was PID - diagnosis was delayed due to lack of space for a proper exam.

@PfParks You're right @PfParks location does matter. How much less likely is a patient to share a sensitive concern when conversation takes place in a public space? How likely am I to miss a finding when exam is done in a chair? Patients deserve better!

AHS: “No matter where a patient is seen at the hospital site, they will always receive the appropriate treatment.” This simply is NOT true. Our nursing to pt levels are often NOT at appropriate levels. Our teams do their best in untenable situations… reddeeradvocate.com/news/red-deer-…
