Re: traversing the planet to the US, asylum was never intended to be "pick where on the planet you want to go."

@AmyInNHTheReal1 @MaryBowdenMD US DOCs didn't choose the lowly primary care spots. The match is a choice; med students pick the spot they want most. 1400 or so of those spots unfilled, according to recent data, were family medicine and internal medicine.

@I_Luv_Idaho @MaryBowdenMD The Mr went to Wayne State on a "scholarship", where they put him to work at $5 an hour as a research assistant. He'll be the first to tell you he's no brain storm. No US with a bachelor's would be a captive at $5 an hour. Many a foreign student is not here due to talent.

@MaryBowdenMD Come on doc, you can see most of these unfilled spots are in primary care which US docs don’t want. Primary care is the issue. It’s been destroyed and a ton of docs don’t what those jobs. We’ve got a primary care problem.

US MD seniors unmatched: 1,367 SOAP positions available: >1,500 Most unmatched US graduates still secure positions through SOAP. Why did they go unmatched in the first place? Because many apply overwhelmingly to ultra competitive specialties like Orthopedic Surgery, Dermatology, Plastic Surgery, Neurosurgery. These fields are dominated by top tier US applicants competing against each other, not IMGs. In reality, IMGs rarely apply and even more rarely match into these specialties. The claim that IMGs are “taking away” spots from US graduates is simply false. Go to primary care programs across Brooklyn, Bronx, Queens, Chicago, Baltimore and many other underserved areas. You will find residency classes that are nearly 100 percent IMGs, because US graduates often choose not to train there. The truth is simple. NOT AGAIN — Over the past decade, the U.S. has steadily increased the number of residency positions, reaching 43,237 total positions in 2025, the highest ever. Yet even with that growth, the system still struggles to fully align supply with demand. Every year, 93–94% of U.S. MD seniors match, and over 92% of DO seniors match, meaning the vast majority of U.S. graduates secure positions. At the same time, nearly 10,000 international medical graduates (IMGs) matched into U.S. residency programs in 2025, filling critical gaps in the system. Even after the main match, 2,500+ residency positions remained unfilled and had to be offered through SOAP, a process specifically designed to fill vacancies that U.S. grads did not take. IMGs are not “taking” spots at scale, they face much lower match rates (≈58–68%) compared to U.S. graduates, meaning they are competing for and often filling the positions others leave behind. The facts remain — U.S. grads overwhelmingly match at high rates — Certain specialties and locations remain undesirable — And thousands of positions would go unfilled without IMGs Without international doctors, entire parts of the U.S. healthcare system, especially rural and primary care, would be understaffed. How did we end up wanting to blow up everything that adds value to America?

Over the past 10 years, the US has seen a steady rise in foreign medical students taking US student residency spots. Every year, 1000+ US medical students fail to match, while 6000+ FMGs do match. About half of unmatched students find a spot, but they must still compete with FMGs for the leftover positions, leaving 500 - 600 US medical students with a worthless medical degree.
