لو واجهت هذا الموقف و انت في الطائرة و تريد ان تمر و هذا المسلم حاجز الممر حتى يؤدي صلاته، ماهو شعورك و ماهي ردة فعلك!؟ ……..

@DrThawaba @shoaa92 مثلا hypereflexia يتم اكتشاف انزلاق عضروفي عنقي مسبب لضغط شديد على الحبل الشوكي وقد يتسبب بشلل رباعي مستقبلا. أو ورم دماغي مثلا بسبب فحص وجد فرق بين قوة الجهة اليمنى واليسرى وغيرها كثير. الفحص الاكلينيكي لطبيب الأسرة هوا شيء أساسي

@DrThawaba @shoaa92 أتفق معاك 100% د. عبدالله، لو في شي أساسي ينقص نظامنا الصحي فهوا رعاية صحية أولية صحيحة. عدد مهول من المرضى في النظام الغربي يتم اكتشاف أورام وغيرها (غالبا مايشتكي المرضى من أعراض عامه، دوخة أو ترنح أو عدم الإحساس بصحة جيدة. ولكن بسبب الفحص الإكلينيكي الكامل مثلا

أهلا د.بشائر المفروض أن تكوني ضد عمل أي فحص مختبر وأشعة دون سبب كما يفعل العديد من الأطباء لاختصار الوقت وتجنب الفحص الكامل سريريا، لا أن تكوني ضد فحص المريض بيدك وأخذ سيرة مرضية كاملة خصوصا بأنك طبيبة رعاية أولية. جملتك صحيحة لكنه حق أريد به باطل من غير المنطقي أن أفحص cerebellar signs عند مريض يشتكي من ألم في ركبته لكن قد يكون ذلك ذو فائدة لمريض يشتكي من ألم مزمن في الأذن أو مريضة لديها صداع مزمن. نصف مرضاكِ chronic back pain ولا اعتقد أنك تقومين بفحص الdermatomes ولا sphincter tone ولا تفحصين الpulses لاستثناء PVD ، يستطيع الاستشاري أن يعقّد حياتكِ لأن الطب معقد فعلا لكن الوسيلة الأفضل هي أن تبدأ حياتك المهنية بفحص سريري أكثر وسيرة مرضية أكثر رغم التعب المصاحب لذلك ثم حين تصبح خبيرا تستطيع اضافة وحذف ما تريد حسب ما ترى. نصف المقيمين لا يعرفون كيف تفحص inguinal hernia Ascites Lymph nodes examination Scrotal examination Proper foot examination Proper neurological examination فقط لأنهم قرروا أن ذلك لن يفيدهم ولم يركزوا عليه في كلية الطب والامتياز. وحقيقة اخرى رأيتها بأم عيني وهي أن الطبيبة تتجنب عمل ذلك فقط لكونها طبيبة: انا female ما بسوي DRE ولا بسوي urinary catheter insertion وكنت اقبل ذلك وأفعله بدلا منهم ظنا مني بأن ذلك فعلا قد يكون محرجا لها، حتى رأيت بأم عيني worst urethral injuries when she inserts catheter with no experience ورأيت المرضى الذين لم تفحصهم ب DRE وعادو ب rectal cancer عدة سينتمترات من الanal verge فقط لأنها قررت أن ذلك الفحص لا يناسبها كطبيبة "قد الدنيا". الطب لا يفصل على راحة الطبيب وجنسه كذكر او انثى ولا يستوجب التفريق ذلك فيما نفضل أن نفعل من دونه. والجملة المزعجة الشهير " أنا رايحة breast endocrine " لا تعفيك من عمل ما يتوجب عمله وتعلمه قبل إنهاءك للتدريب. (الكلام بالتحديد لمقيمات الجراحة هنا وليس لك) بالتوفيق، هدفي هو أن لا تفهم رسالتك خطأ لأن عدد الاطباء الغير أكفاء يزيد وعدد المرضى يزيد وأنا تعبت. 🤙🏻

My wife had a brain tumor removed this weekend. She’s home now and recovering. Successful surgery and good prognosis. We want to sincerely thank doctors David Clarke and @BalgeesAjlan, along with all the nurses/staff in the QEII neurology department. You folks are awesome.
