Echo for fellows and echo students. A 35 year old male admitted with acute heart failure following an episode of chest pain. What is the diagnosis?

طبعا عدد المستشفيات و الأطباء و القطاع الطبي عموما اقل بكتير أنه يقدم خدمه مميزه للشعب ده كله . و طبعا العدد غير كافي الميزانية غير كافيه و طبعا مافيش سراير تكفي الكل و مافيش علاج كافي
الاسهل علي الأطباء أن يكون عددهم كاف و سرير متوفر بدل الفرك الكتير و الشتيمه و الخناقات

كذا سبب عموما أنه بيبقي مقتنع أن في أماكن و أن الدكاتره مدكنه عليه و ان لو له وسطه هيدخل و بيحاول يستبدل الواسطه بالصوت

شوفت فيديو دلوقتي لواحدة مصورة الدكاترة في مستشفى الدمرداش وبتزعقلهم علشان مش لاقية مكان لبنتها ف المستشفى،ودائما وأبدا بيبقى عندي سؤال واحد بس،هو ليه دايما الشعب بيصب غضبه على الدكاترة في حاجات مش بإيد الدكاترة ؟؟ وبمنتهى العقل والمنطقية لما ميبقاش في سرير لعيان الدكتور ماله؟

اقتناع ان في تقصير في المنظومة الطبيه و أن الاطباء شغالين بذمتين و موفرين مجهودهم للقطاع الخاص. الفكره بقي أن المجهود ممكن يكون في الحكومه اكتر من الخاص لكن المستشفى الخاص الاماكن فيه متوفره دائما لأن اللي بيقصدها عدد أقل .

For fellows. This patient had bicuspid aortic valve and severe AS planned for surgery. 10-15% pts with BAV may have coarctation also. What are the added physical finding you search for coarctation in a given case of severe AS, whatever be the age of the patient.

Answer: 1st ECS: SR. 2nd ECG: ST with steep ascending STDs. 3rd ECG 2: sinus beats, the rest ist AT. 4th ECG: sinus beat, then PJC, vagally mediated sinus arrest with JEB, sinus beat, then junctional escape rhythm. Last ECG lokks like vagal overshoot during recovery period

See the pulsatile swelling above left clavicle. Patient is Asymptomatic after AVR now. His X-ray is shown in this video, echo probe is placed above medial end of left clavicle.
