@gordonhmuir Still HoLEP as a day case in <100g prostates is a maximum desobsrution in elderly not bothering from RE, who will not need further intervention

Very good data. But why do a HoLEP under 100g? Unless the man really doesn't enjoy ejaculating, and demands a guarantee of no further day case or local anaesthetic procedures. I don't see so many of those men.

I found time to prepare this unedited video of a HoLEP of a 400 cc prostate I performed recently in Sofia, Bulgaria. I used a virtual basket setting at 2J 50 Hz... hope you enjoy it. youtube.com/watch?v=fPPsT4…, I reflect on how to approach these extreme cases.

@fgomsan Great videos! The concept of voiding cystoscopy gives good visualisation of BOO, eventually in doubtful cases 👍

@fgomsan Quite an impressive speed removing such volume of tissue👍surely with lumenis morcellator could have taken much longer.. Have you video it?

Tough case this morning. We extracted 360 g of tissue. 1 hour en bloc enucleation, 1.30 h morcellation…

@fgomsan @Dr_Abnormal @quanta_system Still, 1.5hrs for morcelation is fast. It would have been much longer with Lumenis.. did you have to use sometimes bi-polar to resect hard nodules? Or, could you suggest leaving the enucleated hard tissue for , let’s say 7/7 to soften and come back?

400 g En bloc HoLEP today… 1 h 15 min enucleation and 1.30 h morcellation, lots of hard nodules, tough case. Home tomorrow without a catheter after long term catheterisation. @quanta_system

@RIRS_inDelhiNCR @fgomsan @amy_krambeck @LumenisU Weight on specimen from pathology report differs widely with the weight on the pod post HoLEP. It can be heavier once staying in the formalin, but more commonly is lighter? Not sure what’s the correlation. Could be studied..

@RRGonzalezMD @FCAFigueiredo @HMethodistUro I did twice salvage HoLEP post urolift and it was tricky morcelation to not break the jaws of the lumenis 🫣

Challenging Urolift salvage case. 1st pics: obstructed view of prostate w prior Urololift x 4 (70 ml, no IML) came to us w gross hematuria. Adherent bladder stone. 2nd pics: whole implant was nidus for stone (capsular tab in bladder w prox migration of clip). #holep #TURP #PVP

@fgomsan @RaiBpn Busy schedule. How do you avoid back ache after consecutive HoLEPs? Special stool with back and arms?

@RaiBpn I tend to do 60-100 HoLEP cases per month. I operate in different hospitals and they gather many patients that ofter we operate in intensive sessions. Usually operate 4 days per week .
