It was a pleasure to host our Western Canada CTO/CHIP rounds, featuring our guest speaker Dr. Mauro Carlino (@MauroCarlino3) to talk to us about the history of contrast modulation and #HDR in #CTO and #CTOPCI. We were joined by @realarainmd, and it was a great discussion about #HDR and #contrast1st concept. We reviewed 3 complex CTO cases from across Canada and had a fruitful discussion with #HDRanalysis. A fantastic online meeting with excellent turnout and a massive Canadian Interventional Cardiology audience. Would like to thank everyone for their support, involvement and interaction. Thanks Ed! This patient had acute admission with ACS and VF arrest. Preserved LV. As workup for arrhythmia MRI done indeed confirmed no scar. Also ongoing chest pain following failed initial procedure. So territory was alive!Unfortunately perforations are always a risk with these no matter how meticulous you are. It’s not only the wire but the MC passage that can do it. Just as for all #CTO PCI or any #PCI, you try to minimise those risks as best you can. So my approach: 1. Understand the collateral anatomy; 2. Respect the collateral; 3. Be patient; 4. Operate in your comfort zone; 5. Don’t try too hard!

Sometimes #CTO and #CTOPCI skillset are needed to address acute situations. Here inferior NSTEMI and resuscitated VF arrest. Severe distal RCA disease and faint distal collaterals. Initial failed PCI to RCA with vessel closure and large hematoma. Referred for redo attempt at day 3. RCA was still occluded. Very large hematoma limited antegrade attempt (#AWE, conventional #ADR). Even difficult to #STAR. Forced to go retrograde. No septal connection. High risk D1-PLV epicardial. Persistence and patience with Suoh 3!. Eventual Gladius MG to antegrade GC. Routine externalization, IVUS and 4 DES to complete the job. Maybe #TDADR could have been an option here and obviate the need for a higher risk epicardial approach. @DrBIqbal Great case Billy, clearly rota a viable option to disrupt the proximal cap and track SI. Distal cap at bifurcation. Great job in preserving PLV and PDA. Was it provisional stenting at the end ?

Rota can help facilitate SI entry in calcific CTOs. Here RCA CTO. Impenetrable prox cap at RVB origin. BASE not working! Rota wire to RVB and drill! Then Gladius passes easily into SI space. Can’t redirect into PDA/PLV to do STAR!! So retro via LCX epicardial to finish the job.
