A 46-year-old patient presented to a clinician to request initiation of estrogen therapy. The patient’s sex had been designated as male at birth. The patient had been raised in a male gender role but identified as a transgender woman and used she/her pronouns. She reported that she had felt different from her peers for as long as she could remember, at least since she was 5 years of age. She had had increasing distress as her body changed in puberty, which she tried to ignore. When she started college, she explored her feelings more and came to understand her gender identity to be female. She acquired female clothing, which she would wear in private. She also sought out transgender clubs. However, she was concerned about her ability to lead a full life as a woman, fearing rejection by society and her family. She therefore returned to presenting full-time as a man in public. She later married a cisgender woman and is the biologic parent of a 3-year-old boy. Over the years, she had worsening gender dysphoria and related depression and anxiety. She had recently connected with online support groups, and she decided to seek care to discuss initiation of gender-affirming hormone therapy.
