Your patient’s MRI might be “abnormal”… but that doesn’t mean it’s the problem. This landmark study (Brinjikji et al. 2015) looked at >3,000 asymptomatic people. What they found: → Disc degeneration in 37% of 20-year-olds → 96% of 80-year-olds → Disc bulges in 30% of 20-year-olds → 84% of 80-year-olds → Most “degenerative findings” increase steadily with age And importantly: These people had NO pain. But here’s the part most people miss: This doesn’t mean imaging is irrelevant. A follow-up meta-analysis (Brinjikji et al. 2015) found: → Several findings (disc bulge, extrusion, degeneration, Modic 1) are more common in people with pain → The association is stronger in younger patients (<50) → Some findings show no clear relationship with pain (e.g. many annular fissures, stenosis)

If you are a healthcare provider who treats back pain and don't ask for medical history including renal/kidney stuff: Start now. Understand renal function Know renal referral patterns Check/communicate with GPs Take BP Educate patients You could be the one to spot kidney pain.
