There are 5 factors that affect pricing in US: 1. No Central Price Negotiation. In Europe, national health authorities negotiate prices directly with pharmaceutical companies. In the US, private insurers can’t centrally negotiate. 2. Patent and Market Exclusivity. US allow longer patent protections, market exclusivity and patent strategies like evergreening (a way to extend your patent lifetime. 3. Profit Driven Health Care System. You have multiple layers of profit. Pharma, insurers, hospitals, etc. Most of Europe have public health care which removes several layers of profit, as well as system does not gain from high pricing. 4. Marketing and Direct-to-consumer ads. Not allowed in Europe, while in US Pharma invest heavy in marketing ads. 5. Insurance Structure. High level of co-payment on medical purchases. None of this benefit Europe on behalf of US. US have built this system themselves. It’s easy to fix, but would be quite a fight with the lobby organisations.
