I am certain Doctors are purely used in the guise of a “training programme” that involves rotating in hard to fill places/rotas/depts- to do the work no one else can, will or has liability to do. The carrot of training and future good pay used to be enough to quell some. Not now.

"I think it is scandalous that the NHS is allowing the substitution of doctors by individuals without sufficient medical knowledge instead of the obvious response to doctor shortages: hiring more doctors." Dr Mel Ryan, BMA lead on preventing doctor substitution. Advanced practitioners are being used to cover doctor rota gaps across the NHS. Read more: theguardian.com/society/2026/a…

New paper on PAs has dropped. Arguably better conducted research than similar in past. Points to what a complete dog's dinner the PA project has been. No planning. No regulation. No governance. Much to pick over. link.springer.com/article/10.118…

‼️GPs refer for a reason. 🚨Letting non-doctors reject those referrals risks delays, missed diagnoses, and unsafe care. This looks less like efficiency and more like shrinking waiting lists on paper. Patients need timely access to the right expertise. pulsetoday.co.uk/news/referrals…

"very generous" I'll tell you what Tom, do a night shift with a doctor and then tell me being paid a few quid more per hour is very generous. Also it's not "overtime". It's mandatory work as part of rotas that can be up to 72 hours a week.
Tom Calver@TomHCalver

First: resident doctors want “pay restoration” to 2008 levels. Except they’ve already achieved it: their base pay is the highest it’s ever been. They are only down at all when we consider that they used to do vast amounts of very generous overtime on the 2002-16 contract 2/6

@agemjourney An Advanced Clinical Practitioner will also be educated to level 7 - the same as a resident Dr but have years of experience in their role. Perhaps have more respect for other members of the MDT.

It is wild to me that a doctor has to go through 5 years of med school and years of post grad training to see patients independently yet a trainee ACP can start seeing patients on their own + make decisions whilst doing their 2 year part time course...honestly blows my mind..

Physios and nurses (ACCPs) can now run the intensive care unit with NO ICU consultant on site, relying only on REMOTE supervision. The public should advocate for their critically unwell relatives in such an unsafe environment.

The question that @wesstreeting needs to answer Why is he still wasting MILLIONS OF POUNDS training PAs at uni plus giving them all a £5000 bursary - £5,000,000 per year group. Employing them at £47, 800 each (55k in London) When their work must be repeated by a Dr?

@DarrenA2833 @ItsnotrightUK @JupiterRose2016 @DrRosena If that’s the case what’s the point of a PA?

Ok this is just ridiculous. The NHS is deeply unserious. There should be no world where a Neurosurgeon who has completed all their exams is paid less than an art therapist.
ExplosiveEnema@ExplosiveEnema2

Picture 1 Consultant Level Arts Therapist £80-90k Picture 2 Senior Resident Neurosurgeon who has completed all of their consultancy exams £65-67k Resident Doctors are incredibly undervalued Their request for pay restoration is a bargain!

Hi @wesstreeting. Given you have just offered UKGP from this application cycle, implying it is possible and can be done, and since it’s a measure which requires no funding, can we presume this is something you will do anyway as health secretary, irrespective of strikes?

Imagine if RCEM put as much effort in supporting 1000s of doctors who cannot get into Emergency Medicine training or pushing for those who are to have better training and pay. Yet further evidence of their push to replace doctors with ACPs. Pay ACPs more = less £ to employ EM Drs

People need to understand that when they say their training is equivalent to medicine this simply isn't true Drs sitting the same assessments as other will blow them away That's why your distance learning, open book ACP training is not enough to work as a Dr at ANY level

Non-PHEM trainees still had a better percentage pass rate compared to paramedics with that difference being even more apparent with PHEM trainees.
