Today I saw a patient whose notes read
" He could climb the stairs in railway station to catch his train on the platform 3, with a suitcase easily 9 months back. Now he gets breathless with a small backpack"
His cardiologist had written such beautiful, detailed functional assessment.
Simple variables like BP, pulse rate etc have their place - but are often reductionist. That's where a clear description of fitness /frailty comes in.
The cardio could have written NYHA class and called it a day. But he made it so vivid that anyone can understand. And in a beautiful handwriting. Reminded me of my cardiology Prof JBC.
Doctor goals : write such beautiful case history that another doctor is so impressed that he shares it online 😍

@dru_jlh Got to go on the portfolio. Relevant, wanted, multi specialty working, use it for out or region, uk, Europe engagement. Redo as a multi site project next year use the regional St training group. Name and shame IT depts. Local nurse/na feedback on bedtimes as Santa nears

@NEndoscopy @drkeithsiau There are others for observation the side that doesn’t move is sick chest, or sore abdomen, or broken ortho (but the won’t notice till the x ray ) (me)

Not sure what this is fully based on but flipping the pyramid is not better eg straight to scam means nothing without history at the very least…

@NEndoscopy @drkeithsiau Stop having a spat you 2!! I was told it was Osler at 90% hx 10 exam ie pre ix (Osler is quoted as saying something to the effect of: ""Listen to your patient, he is telling you the diagnosis"," and that 90% of the diagnosis comes from a careful medical history)

@DouglasAdlerMD @Merit_Endotek Any covered stent if it’s esophageal uncontrolled bleeding. Great rescue rx. But you’ve still got a sick patient to get to stent removal and definitive rx

Monumental, uncontrolled bleeding from esophageal varices s/p banding 1 week ago at OSH. Mass transfusion protocol, pressors, shock.
Tamponade of distal half of esophagus with a 23mm wide Endomaxx stent to good effect.
