There is money in the NHS but it’s not going to patients…. Years ago when I first worked as a physio there was no private organisation in health, every penny went on clinical care. Then it started. First with the cleaners. 1/5

We need to be aware of this when we use GLP-1 in older adults. Do we need to consider increasing resistance exercise and muscle mass before treating? (This is just my thought, not the words of the author)

When I post reservations on #AssistedDying, the response from its supporters is always the same: choice. As if nothing more needs to be said. I’ve heard the same from Parliamentarians. It’s a good argument, individual choice matters, but does it outweigh everything else? 1/6

@wesstreeting Please review the medical exemptions. People with an underactive thyroid get ALL their prescriptions free, related or not. People with asthma - which kills - don’t even get their inhalers. It’s a joke. Change medical exemption.

Last year we froze prescription charges for the first time in years. We’re freezing prescription charges AGAIN - and we’ve kept all the exemptions for those who need them.

Thank you for all your supportive messages. I didn’t expect to be jumped upon for following evidence based medicine and I have muted thread for my own sanity. I will always try to do the best for my pts and try to guide them towards what they need following evidence of experts.

@Molly2323232323 @parthaskar Exactly. Doctors won't understand in depth the particular spectrum of knowledge and skills of other professions. All professional individuals are accountable to the standards of their own profession. Supervision of non-professionals would be a task involving significant resource

@parthaskar But those who claim their role is 'advanced practice in the base profession' also claim to work independently & autonomously So no supervision should be required If supervision and/or oversight are required, this should come from those in the base profession NOT DOCTORS

Lesser trained people (aka ‘Differently trained’ as per some) runs high risk of safety issues UNLESS the supervision is good and effective Problem? Anyone who tells you that senior clinicians have enough time in their job plans to supervise effectively? Either doesn’t work in the NHS Or are lying.

I have retired from seeing patients now but most days I think about it I think about what I got wrong. Usually in the diagnosis Diagnosis is difficult. Algorithms pick up common presentations. Rarely is this real life. This is why training, experience and supervision are vital

It had to happen. I've kept myself safe through all four months of the house ridiculousness, carpet cutting and carpet gripper removing. 'Fell' (lost balance) on the stairs coming down from coffee with a friend. Pride dented. Bum bruised. Knees and shoulder jarred. Ouch.🤦‍♀️

@alisonleary1 And yet research shows us that end user involvement has much greater success rates. So why don’t we do it? Is it because we don’t ask the questions that we know we won’t like the answer to?
