The emphasis of the OP is not on the title (hence, the square quotes). All elders are equal in authority. But they are not equal in influence and responsibility. Some elders simply have more influence in both the elder room and the congregation because they have more influence and responsibility. They spend more time than the others thinking, studying, meeting, planning, leading, organizing, etc. Good! While they do not have more authority than other elders (ever vote counts the same), they do have more investment, practically speaking. This means that some elders carry more "weight" in vote-shaping discussions. Denying such a reality exists on elders boards is naive, at best. Use whatever titles you want, or not. But unless every elder in a church is equally a "double honor" guy with the same amount of influence, responsibility, and renumeration, the reality described in the OP is inevitable. Nobody does any good ignoring or denying its reality simply because it doesn't comport with a narrow and wooden application of Scripture. If they do, they a) are really naive, and/or b) has never served on a elder board in any capacity. Moreover, titles like "Senior Pastor" or "Lead Pastor" or "Teaching Pastor" are simply a shorthand way of acknowledging this dynamic. Some titles may be more or less helpful than others. I'm happy to admit that. But, ultimately, whether to use one title over the other, or no titles at all (as my Alexander-Strauch-influenced, Plymouth-Brethren-adjacent friends may prefer), is a debate over Christian prudence. On this point, my church's confession acknowledges that "there are some circumstances concerning the...government of the church, common to human actions and societies, which are to be ordered by the light of nature and Christian prudence, according to the general rules of the Word, which are always to be observed." Distinguishing "double-honor" elders from other elders by the use of particular titles is simply prudential in this way. Given this, the one or more "double honor" men on the board need to be aware of this dynamic and serve the other brothers who don't have their "hands to the plow" in the same way (quantitatively speaking), so they don't functionally undermine the equal and shared authority of others.
