Tesamorelin is an FDA-approved 44-amino acid GHRH analog that stimulates natural growth hormone release, reduces visceral fat, and supports metabolic health without direct HGH injections. What Tesamorelin Is Tesamorelin is a synthetic peptide analog of growth hormone-releasing hormone (GHRH), consisting of 44 amino acids with a trans-3-hexenoic acid modification that enhances stability and resistance to enzymatic breakdown in plasma. Unlike direct human growth hormone (HGH) injections, tesamorelin works upstream, signaling the pituitary gland to release growth hormone in a pulsatile, physiologic pattern, preserving the body's natural feedback mechanisms. It is sold under the brand name Egrifta and is the only FDA-approved GHRH peptide. Mechanism of Action When administered subcutaneously, tesamorelin binds to GHRH receptors in the anterior pituitary, triggering the release of endogenous growth hormone (GH). This GH then stimulates the liver to produce Insulin-like Growth Factor 1 (IGF-1), which mediates many of tesamorelin’s anabolic and metabolic effects. Dosage and Administration The standard FDA-approved dose is 2 mg daily via subcutaneous injection, typically administered before bed or in the morning on an empty stomach. Newer formulations, such as Egrifta WR, allow weekly reconstitution and reduced injection volume. Benefits Targeted visceral fat reduction without affecting subcutaneous fat, Improved body composition and lipid metabolism, Enhanced IGF-1 levels supporting anabolic and recovery processes. Safety and Side Effects Tesamorelin is generally well-tolerated. Common side effects include injection site reactions, mild joint pain, and transient edema. This may be the biggest gamechanger for longevity medicine since GLP-1s. Heart disease has shaped my family. My dad survived a massive heart attack and eventually needed a heart transplant. My grandfather spent the last six years of his life bed-bound after a stroke. Then my own LDL came back at 192. A PCSK9 injection changed my trajectory. Every other week is manageable. Most people would rather take a pill. The FDA just approved Lipfendra, the first oral PCSK9 inhibitor. LDL fell 56–59% vs placebo. ApoB fell about 50%. Injectable-class potency in a once-daily tablet. The biology is not new. What feels new is the possibility that far more people may actually use it. The safety signal so far is reassuring. It is not a perfect pill. You take it in the morning on an empty stomach, then wait 30 minutes. Product-specific cardiovascular outcomes are still pending. Can you eat your sunscreen? Maybe. There's a compound in your pantry that can change your skin's response to light. That is pasta sauce. A ten-week trial fed participants two and a half tablespoons of tomato paste with olive oil daily. The result was 40 percent less UV-induced skin redness than in the control group, and a second trial found it reduced UV-induced DNA damage. The mechanism is based on the compound lycopene, the pigment that gives tomatoes their red color. It builds up in the bloodstream and acts as an antioxidant in skin tissue, neutralizing the free radicals generated by UV light. It takes at least ten weeks of consistent intake for protection to accumulate. Better DoorDash some Ragu. Note: This does not replace sunscreen. But what I find interesting is the relationship between our diet and the environment. One could potentially eat a "sun-protective" diet.

"Off-label" use of testosterone therapy was associated with a 51% greater relative risk of major cardiovascular events. Among 227, 108 men using it, those without documented low testosterone also had higher mortality and more heart attacks, strokes, and heart failure over 10 years. Observational, not causal. And it does not contradict randomized evidence such as TRAVERSE (which I've spoken about before) in properly diagnosed hypogonadism, where testosterone therapy showed no increase in cardiovascular events vs. placebo. But it does caution against liberal use of TRT.

Short explanation: When fatty acids are abundant (e.g., during fasting or high-fat intake), they are preferentially oxidized for energy in tissues like skeletal muscle and the heart. This inhibits glucose uptake and oxidation through several mechanisms: •Fatty acid oxidation increases acetyl-CoA and citrate, which inhibit key enzymes in glycolysis (pyruvate dehydrogenase) and glucose breakdown. •It reduces insulin-stimulated glucose transport into cells. •As a result, the body spares glucose (saving it for the brain) and shifts to burning fat. Conversely, high glucose availability can suppress fatty acid oxidation.
