@BioLayne pie in the sky ques for ya. What is the recommendation from you for a mid 40s mom who barely has time to put matching shoes( lives in a chaos)in terms of protein intake and exercise? I usually recommend try to get 100gms/day and see if you can get 20mins of exercise.

this takes 5 minutes and anyone can do it. This method is as accurate as a DXA scan (!!!) and predicts risk of diabetes and even death from heart disease youtu.be/WlVbeXCMHRI

Fat is not all created equal. Visceral Fat (in & around organs) is the more metabolically damaging kind How can you measure your Visceral fat? MRI or CT scan are good but expensive There is a simple test you can run at home FOR FREE that gives you a good estimate see below🔽

In my mid-50s and just had a yearly physical—here are my labs. 🤷🏻‍♂️ [ also had a complete blood count and comprehensive metabolic panel but they were so completely boringly normal that I have omitted them in the sake of brevity lol ☺︎ ]

My baseline results for my VEGAN-KETO experiment. I will be on an animal-free ketogenic diet for approx. 3 weeks, mostly to see its impact on my LDL-C.

@tednaiman what would you recommend as the best canned small fish - Anchovies/ sardines/herring? Can’t do water as the fishy taste is overwhelming.

@MichaelMindrum @DrEenfeldt @DrNadolsky Michael, aren't you already recommending Fung and Westman on valley metabolic health? Have you ever interacted with Dr. Westman? You should know better. In this day and age, it is very easy to spot these things. Bikman - may be you can learn a thing or two from him.

@Lpa_Doc So, do you expect Lp(a) lowering therapies worsening IR and type 2 DM in patients or at least make it harder to manage with existing therapies for these metabolic conditions? how should once proceed in such cases?

So true, but biology, and life in general, are complex. We need to keep striving to get to the truth. It's not easily revealed, and humans are imperfect at finding real truth despite best efforts.
Surya@sunnylifer

@Lpa_Doc Now what? Low Lp(a) is also a problem? High is also a problem? Current scientific research Can’t come to a common conclusion despite billions into research. All we get is contradicting evidences.

As usual, drugs that lower LDL and raise CAD risk, like torcetrapib, evacetrapib, HRT, niacin and folic acid, or those that raise LDL and lower cardiac risk like SGLT2 are simply ignored or excluded.
Crémieux@cremieuxrecueil

The association between LDL reduction and coronary heart disease (CHD) risk reduction is sizable and confirmed by - Prospective cohort studies - Mendelian randomization - RCTs There really is no room to deny it. You can't get clearer than this:

@ethanjweiss @drjasonfung Ethan, if we are to genuinely put patient care ahead of all the bias and politics, all available treatment modalities that address the major causal factors must be explored with the same vigor not just the ones that benefit big pharma. Let’s forget abt LDL for 1min(not ignoring)

@drjasonfung @ethanjweiss Why won’t they conduct at least a couple of RCTs with LC + Fasting? No profit to be made in it I guess.

@ethanjweiss Hundreds of interventions? I count statins, ezetrol and pcsk9. Am I missing anything else? Not including torcetrapib and HRT and the ultra low fat NCEP step 2 diet.

@Pl4ntbased Despite your 💭 on Dave, you should coordinate with him…design, results reporting a priori - high SFA low fiber arm, to see if you could induce phenotype, then tank SFA/cholesterol, recheck, then add fiber, recheck, sounds miserable but would be be very interesting 🍿

I'm thinking of doing another N=1. I want to know what impact an animal-free, cholesterol-free, low saturated fat, *ketogenic* diet will have on my LDL-C. My only concern is that I will not respond well to ketosis. I usually eat a low(er) carb diet, but not low enough to induce nutritional ketosis.

@ethanjweiss I think the intent is exactly to say that ALL interventions work that way. I’ve heard this argument hundreds of times in lectures and it’s all cherry picked evidence
