Its the inverse of the clinical criteria for switching to oral we've published: IV needed when 1) Hemodynamically unstable 2) Needs source control procedure/persistent bacteremia 3) Gut not working 4) Resistant to oral options 5) Can't afford or won't take orals

@BradSpellberg @JoshOsowicki i am trying to figure out when IV is required and for how long . i see all kinds of osteomyelitis... it's IV and a PICC line

Declarative statement: The predicted target attainment (ft>100% MIC) for cefazolin CI for S. aureus meningitis is better than that of ceftriaxone for S. pneumoniae meningitis. We fear the former>later ID peeps, please discuss @jasonroberts_pk @IDPharmacometrx @ErinMcCreary

36% reduction in urine cultures (although trending down before intervention) and 36% reduction in antibiotic use with no change in LOS, transfer to higher level of care, or mortality using BLADDER score in work-up of potential UTI. cambridge.org/core/journals/…

🆕 in @ICHEJournal - we devised a simple scoring tool to help reduce unnecessary urine culturing and antibiotic prescribing for asymptomatic bacteriuria. The BLADDER score can be used as part of broader UTI stewardship efforts. cambridge.org/core/journals/…

@EricMacyMD @KPSCalResearch @EricMacyMD did you also remove any notion of cross-reactivity within the cephalosporin class? (eg. patient with cephalexin "allergy" prescribed a cephalosporin with the same side chain)

At Kaiser Permanente we have removed all warnings not to use cephalosporins in the setting of a penicillin "allergy", including the very rare history of penicillin-associated anaphylaxis. @KPSCalResearch jaci-inpractice.org/article/S2213-… jaci-inpractice.org/article/S2213-… jamanetwork.com/journals/jaman…

Would love to hear from @EricMacyMD @TrubianoJason @blandman19 Would you consider this case as anaphylaxis with this limited info/time? Do you consider anaphylaxis a reason not to give cefazolin or to be cautious in penicillin allergy? #IDtwitter

@DrToddLee @CIDJournal You're more likely to find a unicorn that is eating four leaf clovers than finding a person who would be eligible for EPIC-HR in 2024

Inspired by a discussion on inpatient rounds this week with super ID fellow @GaboMotoa, here are a couple of great @CIDJournal perspectives that challenge the status quo on infection control practices. First up, MRSA. doi.org/10.1093/cid/ci… 1/x

Anti-vaxxers going crazy over headlines from the @Telegraph misrepresenting an already bad paper from the @bmj_latest just shows what has always been true. Anti-vaxxers don't read.

I had to take a break from the platform, but I missed all the great discussions and info gained from the ID community. After 1 day of pinning @GermHunterMD's #IDTwitter and @sebpoule's #IDTwitter StarterPack lists to my homescreen: I CAN'T BELIEVE I DIDN'T DO THIS SOONER!

Recently had an ER visit for dehydration while out of town. Received some labs and 3 IV bags. Here's the bill- AFTER insurance. It's no wonder medical debt is the top cause of bankruptcy in the U.S. 🏥💸 #HealthcareCosts #MedicalDebtCrisis

“ID experts aren’t so much specialists in starting antibiotics, they are more so specialists in stopping them when they are unnecessary.” It took me a while to understand this. The amount of unnecessary antibiotic exposure that ID docs prevent is immeasurable.
