The prevalence of hypercortisolism was an alarmingly high 24% in difficult-to-control type 2 diabetes. This was based on a dexamethasone suppression test with cortisol > 1.8mcg/dL in the CATALYST trial. Targeting hypercortisolism such as with a glucocorticoid receptor modulator could be a unique therapeutic target for the treatment of type 2 diabetes without adrenal adenomas.
