Over 100 years ago medical education was changed for the better. Abraham Flexner provided a report on the status of American and Canadian medical schools that led to standardization of medical education and dramatically advanced the field. Nurse Practitioners need their Flexner Report. A recent article by @business outlined what many of us in medicine already know…. The field of Nurse Practitioners has been overtaken by those motivated purely by profit who have preyed on unsuspecting nurses looking to advance their careers. Far too many graduates have no experience and are ill prepared for the critical function that midlevels perform in our medical systems. This @BeckersHR article outlines the key problems facing nursing education and I encourage everyone to read and better understand the issue. There is a significant need for physician assistants and nurse practitioners alike given the increased demand for healthcare in the United States, but that need cannot be used as justification to provide subpar education that puts patients at risk. Budget neutrality. There is only one pie and the government pits physicians against each other for the pieces. All subspecialty lobbyist hit congressional offices arguing for their peace while the AMA laughs its way to the bank with the CPT code Monopoly. That's a crude way of saying it but true. Hopefully more recent collaboration in the AMA can breakthrough and stop the vicious cycle.

We absolutely need to worry about the future of specialty care. If I do a craniotomy for a TBI at 2am, the payment system now financially rewards me more for billing the ICU care afterward than for performing the actual surgery itself. We can increase primary care reimbursement without creating these kinds of central planning distortions.

Top-tier #MedTwitter click bait in @Health_Affairs. Here's the thing: 1) We don't need to worry about "sacrificing specialty care." The average specialist makes >$400K. 2) It is entirely reasonable to evaluate the impact of recent payment increases before doing more.

Our medial knees absorb 2 to 4 times body weight through routine walking and 4 to 6 times body weight while running. While Mike is bowlegged (Genu Varum) it's safe to say that example is more so in him. A 10 pound weight loss is going to give a minimum of 50 pound reduction in force through the knees while running. You can pick up a 45 pound plate in the weight room and feel what that is like. Great to see him feeling better and playing better this season. #SportsMedicine @GoldStateOrtho

Yes, no, maybe here. Every free agent set up for a big money deal like that submits all medicals and MRI elbow, shoulder, lumbar spine to a potential team for review. Elbow arthroscopy or not, the elbow will be thoroughly evaluated prior to signing a big free agent contract. While loose bodies can be concerning for valgus extension overload and potential longer-term problems, people would be amazed what these athletes live with and can perform with. Let's hope this phenomenal player can get back to doing what he loves soon.

There is nothing to debate on E-Bikes. That is the most bipartisan issue out there. They need to be regulated and the laws enforced. The orthopedic injuries alone are the tip of the iceberg in the tragedy physicians see on the front line following collisions.

This can’t be real… Secretary Becerra administered a patient protection bill so poorly that he was sued four times and lost all four cases in federal court. Has that even happened before?!? Who sued him? The Texas Medical Association. Why was he sued? Because he wrote anti-physician, anti-patient regulatory language that was at direct odds with the actual words in the patient protection bill that Congress passed Who did his illegal language help? insurance companies. That’s right… he put insurer profits in front of patients and physicians I don’t live in California and have not spent time dissecting each of the candidates in the California governor race, but making a statement that the Secretary has demonstrated a commitment to access, protecting patients, and supporting physicians couldn’t be further from the truth based on his last four years in office. @IndeMedAction

To be clear, there are bipartisan bills that move through Congress every year to help clean up healthcare delivery. They never get to the finish line. At this point, unfortunately, the train is too far out of the station and about to go off a cliff. Physicians voices are neutered by the consolidation and practice employment that came from the ACA. Also, by the reimbursement ceiling being lowered while the overhead floor rises. Think of a basement slowly flooding. The water is almost overhead.

This is a pretty solid explanation of the state of our commercial insurance healthcare system. Both sides of the aisle are to blame for letting it get this way. The affordable care act which was well intended to capture uninsured lives has been like gas on a fire.

@leftcoastbsbl 💯 Just look at the example of Cal and Stanford. Two top academic institutions flying their student athletes around the country. Can a science major have a class with a lab? Five trips a year on a chartered flight in football is so much different than every other sport.

Go look at the record books of any non revenue sport. Before the last 10-20 years almost all games were regional. No need for tennis, golf, swimming to travel to Hawaii, Arizona, Florida and Texas all in one year. Play local and shrink the conferences back to what they were and the expense side will take care of itself. There should be football conferences and then everything else.
