"Not to replace you, but to copilot with you." 🤝 Reflecting on a fantastic week at EACVI 2025, this sentiment from Victoria Delgado(@VDelgadoGarcia) , President of the European Association of Cardiovascular Imaging, perfectly captured the mood of the conference. During our workshop, she highlighted how AI is not about competition. It is about support. "These tools in the future will make our lives better, elevating our practices." We saw this clearly in the discussion on inter-variability. Professor Masao Daimon joined the conversation, noting the global shift towards these solutions: "AI tech has gained popularity in Japan and I would like to use it." Clinicians are looking for tools like EchoConfidence because they reduce variability between readers. Atherosclerotic #plaque regression has been well documented under intensive medical therapy. What remains less clearly defined is how advanced imaging can be used to prospectively identify and guide plaques selection of plaques most likely to respond to targeted pharmacological intervention. In our recently published case, we used #CCTA-derived plaque morphology not just as a diagnostic snapshot, but as a guiding tool for therapy. Much like #CT-guided PCI helps us select the right patient and the right device — soft plaques for scoring or non-compliant balloons, heavy calcium for advanced calcium-modifying tools — a CT-guided lipid-lowering strategy may allow us to select plaques that are more likely to respond to pharmacotherapy before they declare themselves clinically. In this case, we move one step further: a carefully characterized plaque, despite being functionally significant, regressed and became flow non-limiting under optimized medical therapy alone. Whether this approach could become a viable alternative to DES or DEB #PCI in selected patients remains an open question. But the concept is intriguing: treating plaque biology with the same precision we already apply to coronary anatomy. We also highlight several plaque-specific characteristics in the discussion that may have facilitated this favorable outcome — factors that could help identify patients most likely to benefit from this strategy. This is not a call to abandon PCI. It’s an invitation to reconsider when and how we intervene — and when biology might still be modifiable. Day two at ESC Digital & AI Summit 2025 started with a sharp keynote from Folkert Asselbergs on the real frontier of cardiovascular AI. He outlined the shift from simple generative models to agentic and multi-agent systems that can retrieve data, reason, and support complex decisions — even integrating imaging, guidelines, risks and patient preferences. Powerful examples, but paired with equally strong warnings: data-poisoning risks, algorithmic bias, and the need for strict validation frameworks. His message was clear: AI in cardiology is accelerating fast, but must stay grounded in guidelines, rigorous evaluation, cybersecurity, and workforce education. First day at ESC Digital & AI Summit 2025 closed with a packed keynote session on one of the most urgent frontiers in our field: how to responsibly unlock the power of AI for healthcare. Max Tschochohei set the stage with “AI decoded”, a clear-eyed tour through what AI can (and can’t) do today. The thread running through his talk was practical empowerment — how researchers, clinicians and health-system leaders can use AI as an amplifier of evidence, not a replacement for expertise. The panel that followed, chaired by Folkert Asselbergs and Thomas Lüscher, pulled the discussion back to reality: governance, cybersecurity, data stewardship, and the messy human factors that shape every clinical workflow. The energy in the hall was unmistakable. If the goal of day one was to align ambition with responsibility, this keynote nailed the landing. #ESCAIsummit2025 #DigitalHealth #AIinCardiology
