@drmhuss We pre-populate on-calls and weekend shifts, to ensure equity, then self roster the gaps (and swap on-calls/weekends if necessary). Sit in a room together for a couple of hours every 4 months or so to sort it. Works well for FT staff and LTFT colleagues with second jobs etc

@OrthopodReg This paper popularised Biers blocks: Kendall JM et al. Haematoma block or Bier’s block for Colles' fracture.... J Accid Emerg Med 1997; 14(6): 352-6. We don't use Biers now. Haematoma blocks work with good patient selection and operator skill. Otherwise sedation or ?penthrox

@goldstone_tony Genuine question. Is there anyone inside HMRC who can actually do the calculations? Asides from the automatically generated brown envelope bills, is there any evidence that HMRC are actually working about of this out themselves?

Complexity is perhaps the biggest problem here. I didn’t even know what AA was until a colleague came with a brown envelope. I looked it up, did a few spreadsheets, and realised it was a massive threat to the NHS.

4) If we take top-tier consultant pay band and apply annual wage increase of 2%, then apply this morning's 9% CPI rate for 2022/23 Growth in 2015 scheme is not impacted as badly as we saw on GPs If we repeat a high jump in CPI in 2024/25, then there's an AA breach

Interesting times on my ED and GP combined rotation. Our ambulance notes are being read far more often than I would have ever believed. I have even seen a lot of ED docs copy and paste directly from our notes to theirs.

I know statistically and intellectually it cannot be possible, I know it's for the best, and I know it's my own 'fault' for not wanting to leave the competitive deanery I've lived in for 11 years, but I can't shake the feeling that I'm the only one who didn't get an ST4 number 🤦‍♀️

@gdcuk @goldstone_tony @JosephineCumbo Not just LTA and AA though. Workload volume and intensity, sub-inflation pay awards, 60% marginal income tax rate etc. Makes increasingly little sense for financial and health reasons to continue to retirement age.

@JosephineCumbo Must just be a coincidence that as lifetime allowance and annual allowance became more punitive for doctors, more doctors started retiring early 🤔 Think someone might not be being given the right data before answering questions
