@drmhuss We pre-populate on-calls and weekend shifts, to ensure equity, then self roster the gaps (and swap on-calls/weekends if necessary). Sit in a room together for a couple of hours every 4 months or so to sort it. Works well for FT staff and LTFT colleagues with second jobs etc

@OrthopodReg This paper popularised Biers blocks:
Kendall JM et al. Haematoma block or Bier’s block for Colles' fracture.... J Accid Emerg Med 1997; 14(6): 352-6.
We don't use Biers now. Haematoma blocks work with good patient selection and operator skill. Otherwise sedation or ?penthrox
English

I’ve just registered that the BOA distal radius guidelines recommend a Bier’s block for ED manipulation, rather than procedural sedation…
Wut?! Really?!

@KineshPatel @goldstone_tony I've often wondered this too. Does HMRC have any idea what I should be paying in AA charges?
English

@goldstone_tony Genuine question. Is there anyone inside HMRC who can actually do the calculations? Asides from the automatically generated brown envelope bills, is there any evidence that HMRC are actually working about of this out themselves?
English

@gdcuk Thanks for this, good to have a rough idea! Will there be much impact on 1995 growth? Or might higher inflation reduce growth (do I recall that only the opening value is increased by CPI)?
English

1) Quick update thread on CPI disconnect in NHS pension, following a great question by @drsas27 asking if hospital doctors are at risk too
Previous thread showed how this is catastrophic for GPs now twitter.com/gdcuk/status/1…
Here's how this could impact ALL members in future

1) I've been asked about -ve growth This is particularly relevant to GPs, but can apply to all Previously shared a quick calc showing impact of CPI and potential for -ve growth👇 @gdcuk/impact-of-cpi-on-gp-annual-allowance-1gO4Hj5fTViSgKb0Dzwksg?s=eyJHUCBDUEkgRmlndXJlcy54bHN4Ijp7IlNoZWV0MSI6eyJGMiI6MC4wN319fQ==" target="_blank" rel="nofollow noopener">grid.is/@gdcuk/impact-…
This short thread explains the maths behind it @nick_grundy English

@davehartin You can add Taunton to that list..
We have fantastic ACPs and tACPs.
English

I don’t know what ill informed and prejudiced commentary had led to this statement being issued but if you don’t fancy working with fantastic ACPs and tACPs, just don’t come to Ipswich.
Clear?

@Lukeslifeonline Jealous. Prehospital notes extremely valuable. I can only access them on 1 PC in my ED and have to print them from this... no cut and paste! @NHSDigital
English

@bethicillin Presumably this means you won't be rotating away? Which is excellent news for us downstairs and our patients. Do let us know if we can help...
English

@gdcuk @goldstone_tony @JosephineCumbo Not just LTA and AA though. Workload volume and intensity, sub-inflation pay awards, 60% marginal income tax rate etc. Makes increasingly little sense for financial and health reasons to continue to retirement age.
English

@JosephineCumbo Must just be a coincidence that as lifetime allowance and annual allowance became more punitive for doctors, more doctors started retiring early 🤔
Think someone might not be being given the right data before answering questions

English

Govt minister concedes the number of the #NHS's most senior doctors (consultants) taking voluntary early retirement has increased since 2012, but does not link to #pensions tax issues. #NHS #taper #AnnualAllowance

Feeling very fortunate for the fab care given to us by the ED, surgical & breast teams @SomersetFT the last week. Back breastfeeding our gorgeous girl after emergency I&D for breast abscess. What a result!! @EdMorleySmith2 @DrJasonLouis

English

@annakirbybailey @KatherineRCEM I would too! Would be great to give Educational Supervisors at their training.
English

@KatherineRCEM I would love a copy of this for our office - do you know who I could contact?
English

@anna_annabav @MPHAcademy @russell_peek @RCPCH_TA @baby_dr_claire Will try to get hold of a copy we can circulate and use for ES training!
English

@BBCNews This makes no sense. If they see 93,000 patients a year an average day is 250 patients - how can 180 patients be a busy day?

Half-way through my ridiculously hot run this morning and a little old man who was stood in his garden started jogging on the spot and shouting ‘you go girl’....far better than being beeped at by pervy men in white vans!! 👴🏻🏃🏻♀️👋 #sundayrunnig #toohottorun

@ladybakerkent @WeNurses @MusgrovePark Not just easier for patients. I struggle to identify colleagues.
English

@WeNurses Couldn’t agree more! @MusgrovePark it’s too confusing for our patients. I love their engagement in choosing the uniforms so let’s use this NHS wide. #oneNHS

@baby_dr_claire @TessaRDavis @Damian_Roland @RCPCHtweets @jamesgagg @carrie_langley @EstherWilson Expect we're better off than many DGHs as we have a PEM Consultant. Would be good to have more space for children to facilitate this and allow increased Paed SpR / consultant presence. ?Physio

@TessaRDavis @Damian_Roland @RCPCHtweets Wow what a dream eh @DrJasonLouis @jamesgagg @carrie_langley @EstherWilson?!

Key standard: ALL departments which see children AT ALL to have two paediatric nurses per shift, a PEM consultant with dedicated shop-floor time, and a play specialist.
