I want to share what I think about MDs. I love each and everyone that busted their ass, borrowed too much money, sacrificed more than we can ever quantify, in hopes of helping their patients. Medicine is an imprecise science. The uncertainties far outnumber the absolutes.

Activation mapping was helpful in this case with @AllyCarey_4 @BiosenseWebster. I have also seen many cases where entrainment and identification of low amplitude fractionated EGMs that can span the entire cycle length are the key to eliminating atypical flutters.

Redo case today with multiple atypical flutters. Posterior and anterior scar. Limitation to all mapping systems seems to be accounting for low amplitude fractionated EGM’s IMHO. How many rely on mapping system? Entrainment? Voltage? Looking for fractionated EGM’s? #EPeeps

Rarely need Impella for VT. Turned out to be great in this case. Was able to find critical isthmus for clinical VT in patient with VT storm. Single burn termination for that one. No longer inducible for VT. Thanks ⁦@AllyCarey_4⁩ & ⁦@BiosenseWebster⁩. #AblateVT #EPeeps

Triangulation of basal inferoseptal PVC from LV, RV and MCV. Suppressed from LV with high power long duration but with late recurrence. Long term suppression after ablation from all 3 sites. Interesting case today with ⁦@AllyCarey_4⁩ and ⁦@BiosenseWebster⁩. #EPeeps

@vish_luther @AllyCarey_4 @BiosenseWebster I did not do an angiogram. Made sure the catheter tip was pointed toward LV Endo and gave a single 30 second lesion at 30 Watts. If I felt more aggressive ablation was needed, I would have. Certainly would not be wrong to do so.

Complex extraction case with @rdschaller. Recurrent bacteremia with jailed RV dual coil lead & SVC stents. Generator removed previously. Snared end of coil with bioptome and goose neck, then unraveled the ICD coil enabling extraction around SVC stents. #EPeeps @PennCardiology
