If this patient was in Atrial fibrillation, that may be Ashman’s phenomenon on the differential, in addition to the other excellent comments.

@AslangerE ST elev in III>II with depression in aVl and I worrisome for OMI. Although do see pR elev in aVr and v1 for pericarditis. Now pain free= R sided ECG And Serial ECGs? ECHO? Trop trend? Any new symptoms or ischemic results=Low threshold to cath.

Consulted from psychiatric ward due to 2-3 min tingling pain over left breast, in a now pain-free 56 yo f who is treated for schizophrenia. Pts is cooperative but denies any pain now, 2 serial ECGs 15 min apart were identical with the current one below. What would you do ?
