I risk sounding callous when posting this but whose arms do you think they’ll run into when their innocent children develop a deadly vaccine-preventable illness

The Twitter replies to the AAP President’s very tepid statement on RFK Jr’s DHHS nomination are absolutely deranged - just a cavalcade of anti-vaccine nuts calling pediatricians stupid, evil or both. Maybe it’s time to get off Twitter and move to bluer pastures

@AmerAcadPeds The AAP should be firmer in its stance against RFK Jr as DHHS secretary. The American public is not ready for a resurgence of measles, mumps, rubella, polio. Hib, invasive s. Pneumo. This will be the consequence of amplifying the anti vaccine movement within DHHS

First patient of the day was a NB teen that attempted suicide, citing the anti-trans environment promised by a Trump second term. Pulled me right out of my funk - this is why we fight even when we can feel hopeless. Protect trans youth

The images from Al Aqsa Hospital are horrific and gut-wrenching. Displaced families being burned alive. Where is the humanity? This has been going on for over a year now. The U.S. must stop sending bombs and we desperately need a ceasefire.

My son started kindergarten this week. I’m in tears thinking about what this family was going through, and they were lucky compared to some of their peers. It doesn’t have to be this way - we need gun safety legislation now

@aldinaaraul It’s hard because the pediatrician was right to worry about the bilirubin level, and right to insist on phototherapy. But how they took it from there only further contributed to the family’s fear of trauma from the medical system

@EdGillMD I agree with this. However, now people are vilifying the pediatrician. The entire situation was handled incorrectly. Like you said education is important & working WITH midwives as part of the healthcare team instead against.

A lot of people focusing on the bilirubin level (which yes, was high enough that under current guidelines the infant needed phototherapy to prevent kernicterus) when the real tragedy was the way CPS, the police and the pediatrician responded.

It doesn’t mean I don’t want to listen to you but you have dramatically overestimated how prevalent this issue is, and underestimated how much thought and counseling goes into these decisions for people living it vs arguing about it on Twitter

@EdGillMD @benryanwriter @TheEconomist I'm not making it personal but doing so is a classic response to not wanting to listen to someone. Many doctors, responsibly, baulk at vasectomy or tubal ligation for people prior to children/full brain maturation I care about the cohort now identifiying as trans, we all should

@CabsnKings @benryanwriter @TheEconomist I should mention one of the challenges of displaying MH impacts of PBs is that the greater risk factor for suicidality is an unsupportive home environment. Kids on PBs have by definition a supportive home environment (and again it’s a very small # of trans kids even getting PBs)

@CabsnKings @benryanwriter @TheEconomist I mean the systematic reviews on the subject in pediatrics find the data inconclusive on MH and suicidality benefit but pretty clearly demonstrate safety, concluding that more robust RCT are required. That will be stymied by bans and the current climate around this discussion

@CabsnKings @benryanwriter @TheEconomist It sounds like your problems are with adult medicine providers? We let people 18+ make their own medical decisions in the US, I don’t know what to tell you. Do you have a personal connection to this issue, or are you more a hobbyist?

@EdGillMD @benryanwriter @TheEconomist We have a problem here in that there seems to be very little if any central collection of the data. Defs of trans vary. Defs of youth vary - for me this is prior to full brain maturation. Informed consent therefore at PP for 18s does not make sense by my understanding of consent

@CabsnKings @benryanwriter @TheEconomist You’re an empiricist per your bio, are you familiar with the Evidence-Based Medicine Pyramid? Case Series and Case Reports (as you’ve linked) are the lowest level of evidence. (Above expert opinion)
