The alpha-synuclein seed amplification assay was initially celebrated as a disease-specific biomarker for synucleinopathies. This review from Fullam et al. in EBioMedicine complicates that picture in an important way: SAA positivity in CSF occurs across a range of neurodegenerative conditions not classically considered synucleinopathies. The authors systematically examine SAA positivity in Alzheimer's disease, progressive supranuclear palsy, corticobasal syndrome, idiopathic normal pressure hydrocephalus, and traumatic brain injury. The signal in these conditions reflects alpha-synuclein co-pathology — something we know is common at autopsy but hadn't previously been so clearly demonstrable in vivo. The clinical implications are significant. A positive CSF alpha-syn SAA in a patient with an atypical presentation doesn't automatically confirm PD or DLB. It may instead signal convergent pathology that drives clinical heterogeneity. This most recent paper by our champ post doc @VictorGabriel_H @TJUHNeurosurg showed that despite differences in baseline comorbidities and angiographic severity, pediatric and adult patients with Moyamoya had similar perioperative and long-term stroke outcomes after bypass surgery, supporting the safety and durability of revascularization across age groups. I want to thank all our collaborators.
